Licensee Surveillance of Pipe Support and Restraint System a.

Review of Procedures The inspector reviewed the documentation and held discussions with cognizant licensee personnel regarding the surveillance program of hydraulic and mechanical shock st? pressors, and other pine supports.

The following documents were reviewed:

Visual Surveillance Data The inspector re, ewed the visual surveillance data sheets for the following pipe supports and restraints:

SS-1-10, SS-1-10-1, SS-1-10-3, SS-6-10-7, SS-6-10-8, SS-10-30-1, SS-10-20-2, SS-10-20-3, SS-10-20-8

Maintenance Summary and Control Sheet, dated 10/23/81 for Hydraulic Snubbers, MR Log No. 81-55-12.

The data for the above pipe support and restraints were reviewed to verify the following:

-

There was no visible sign of deterioration and/or corrosicc

-

There was not physical damage or deformation to render the support or restraint inoperable.

-

The support or restraints were not in " frozen" or " locked-up" position.

-

Bleed holes were open and free, if applicable.

The inspector observed that out of seventy five snubbers inspected in October of 1981, thirteen snubbers appeared to have failed to meet the visual acceptance criteria.

The licensee is evaluating the effect of such a failure rate on the scheduling of the next surveillance of supports and restraints. The plant technical specification requ!res a shortening of scheduled surveillance and inspection interval to a cycle of thirty one days if the number of unacceptable snubbers in any one inspection cycle is eight or more for any one group of snubbers.

The inspector also noted that the surveillance reports for hydraulic snubbers for March, 1979 and November, 1980 also contained many examples of snubber accumulators indicating apparently empty reservoirs, however, bench test data for snubbar operability were not readily available to determine the functional operability of snubbers in service.

Because

the plant Technical Specification required that the frequency of surveillance / inspection be based on number of snubber failures, the inspector was unable to establish the validity of surveillance frequency.

This item remains unresolved and will be followed-up in subsequent inspections. (50-293/81-34-01)

Visual Inspection of Masonry Walls, and General Walk-through Inspection The inspector conducted a general walk-through inspection of the plant to visually inspect accessible pipe supports, snubbers, and restraints to assess the general condition of maintenance, tagging, and up-keep of safety related items.

The inspector also visually inspected masonry walls which have been reported by the licensee as to not meeting the acceptance criteria established pursuant to It3:80-11. However, the licensee's submittals to NRC in responsa to the bulletin have not been fully reviewed by NRC, therefore, the bulletin remains open pending the review and subsequent inspection of the completed work. The senior resident inspector accompanied the inspector on this inspection.
