Enforcement Conference on October 15, 1981 (Report No. 50-293/81-29)

Summary: Special enforcement conference convened by NRC Region I manageraent to discuss NRC concerns regarding recent inspection findings related to combustible gas control, containment isolation valves and fire protection, and concerns in-volving management control of safety related activities at the station. Senior Boston Edison Co. and NRC Region I management personnel attended the meeting which was held at the Region 1 Office. The meeting was about one and one half hours in duration.

Summary Mr. Haynes opened the meeting and discussed the results of recent in-spections and analyses by NRC inspection personnel which indicated se;eral problems in the management of activities at the Pilgrim facility.

Parti-cular concerns discussed included: 1) recent apparent items of noncompli-ance in meeting the NRC requirements for post-LOCA combustible gas control inside containment and Technical Specification limiting conditions for operation regarding containment integrity and fire protection, 2) the Boston Edison management weaknesses identified during the recent NRC Per-formance Appraisal inspection completed in August, 1981, 3) the large number of man-rems of radiation exposure received at Pilgrim in 1980, and 4) the results of a regional assessment of the licensee's safety per-formance during the preceding twelve months which identified the large number (9) of Immediate Action Letters (IALs) issued to Boston Edison by the regional office, their failure to fulfill commitments and the need for increased attention by Boston Edison management to those activities affecting the safety of plant operatio _

During this discussion, Mr. Haynes stated that the apparent items of non-compliance indicate significant failures in the control of design, modifica-

tion and maintenance activities as well as the owating procedures, opera-tor training, safety committee and management oversight of activities at the Pilgrim facility. Mr Haynes further stated that these noncompliances were being reviewed according to NRC's Interim Enforcement Policy to ascertain which enforcement option would be invoked and noted that esca-

,

lated enforcement action was being considered.

Regarding the management weakness identified during the NRC's performance appraisal inspection and the regional assessment of the licensee's safety performance, Mr..Haynes stated that prompt and substantive corrective actions were required by Boston Edison management. These assessments pro-vided further evidence of the apparent lack of planning, control and management involvement in the maintenance and operating activities at the site, the lack of an effective corrective action system and failure to meet connitments made by Boston Edison management to the NRC.

The licensee's management representatives agreed with the need to improve performance in these areas, stated that they were not satisfied with the results of recent evaluations, and committed to achieve improved perform-ance promptly. The licensee discussed corrective actions to address im-provements in the areas of safety evaluations and field engineering support, corrective actions systems and LER trending, and timely implementation of commitments. The licensee also discussed their activities to integrate the

Company's resources of technical and operating skills and to increase these resources.

The licensee's management also stated that several technical issues were

planned to be resolved prior to plant startup from the current refueling

outage. These items included the RBCCW heat exchanger, drywell high temperature, and masonary wall problems.

l The licensee's management representatives reiterated their policy to conduct l

activities in such a manner as to ensure the health and safety of the public.

The licensee also requested an opportunity to discuss further details of their corrective actions with the NRC at a subsequent meeting to ensure that all concerns are understood and properly addressed.

The NRC Region I management acknowledged the licensee's statements and l

stated that the NRC concerns about the management control and safety per-

formance at the Pilgrim facility would be the subject of future correspon-

dence, and that a meeting between the NRC and Boston Edison Company management

'

to discuss additional details of corrective actions is expected to be held l

prior to plant startup from the current refueling outage.
