The inspection scope and findings were summarized on March 10, 1982,~with those persons indicated in paragraph 1 above. The inspector stated that the health physics job coverage appeared to be excellent and the procedure

changes, recently implemented, appeared to be very effective. The general manager acknowledged the inspector's comments.

Licensee Action on Previous Inspection Findings (Closed) Violation 81-07-08.

This item concerned the failure of the licensee to provide the NRC with individual termination exposure reports

,

within thirty days of exposure determination. Licensee procedure HP-95 has been written, approved, and is now in use. The procedure was examined by the inspector who had no further questions.

(Closed) Violation -81-10-01.

This item concerned the failure to make an adequate evaluation of survey information and the failure to provide ade-quate monitoring devices for steam generator work.

Licensee procedures HP-7, HP-9, and HP-12 have undergone major revisions. These procedures were examined by the inspector and, in addition, actual practices were observed in work areas. The inspector had no further questions.

(Closed) Violation 81-10-03. This item concerned the overexposure of three workers during 1980. In addition to the major procedure revisions noted in the previous item, the inspector specifically observed health physics coverage and dose calculation. The inspector had no further questions.

This item concerned the failure of the licensee to maintain radiological survey records as specified in 10CFR20.401. The inspector made a specific record review. Record retention appears to be adequate. The inspector had no further questions.

This item concerned the overexposure of a worker in August,1981. See 81-10-03.

This item concerned the failure of the licensee to perform an adequate evaluation of survey information.

(Closed) Violation 81-24-04. This item concerned the licensee's failure to provide fully qualified technicians for steam generator coverage. Procedure HP-12 has been revised and Section 5.5.1 requires an ANSI qualified tech-nician.

In addition to the procedure review, the inspector observed actual practices. The inspector had no further questions.

Licensee Action on Previous Inspector Identified Items (Closed) IFI 81-07-03 - This item concerned the improper removal of protec-tive clothing.

Licensee procedure HP-28, titled " Control of Personnel

Contamination and Decontamination Techniques" has been revised and speci-fically covers clothing removal.

Extensive training was given and docu-mented for all plant personnel. The inspector had no further questions.

(Closed) IFI 81-07-09 - This item concerned the lack of comparison between pocket chambers and TLD readings.

Licensee Procedure HP-9.6, titled

" Dosimetry and Incident Investigations", was recently put into use.

Comparisons are now made and investigations are performed as necessary. The

inspector had no further questions.

(Closed) IFI 81-07-11 - This item concerned the lack of a regulated power-supply for the whole body counter. The power supply is now regulated. -The inspector had nc turther questions.

(Closed) IFI 81-07-22 - This item concorned the lack of a routine review of personnel contamination by plant management. Licensee Procedure HP-28 was revised and specifically covers this concern.

In addition to a review by management, a second review is made by the ALARA specialist. The inspector had no further questions.

(Closed) IFI 81-07-31 - This item concerned sample hood face velocities possibly being less than 100 linear feet per minute (LFM). A memorandum from D. S. Crocker, Manager Environmental and Radiation Control (E&RC), to all E&RC personnel, dated December 17, 1981, informed all personnel that hoods are marked at a point that will assure greater than 100 LFM.

The memorandum further states that hoods are not to be in use when doors are fully open. The inspector had no further questions.

(Closed) IFI 81-07-32 - This item concerned an inadequate filter system in use at the emergency spent fuel building.

Repairs were performed on a gasket and the filter system is now in good working order. The inspector had no further questions.

(Closed) IFI 81-07-33 - This item concerned control room ventilation and filtering as proposed in Regulatory Guide 1.52.

The licensee has reevalu-ated the present system and the design is in keeping with the aim of the regulatory guide.

The guide does not appear to require backfit and the licensee is not planning any changes in the present system. The inspector had no further questions.

(Closed) IFI 81-07-37 - This item concerned the lack of a safety analysis for the site settling ponds. An analysis was completed on February 9,1982.

The inspector reviewed this analysis and has no further questions.

(Closed) IFI 81-07-39 - This item concerned the use of only one isotope to calibrate the liquid effluent monitor.

Licensee Procedure HP-42 titled

" Calibration of Radiation Monitoring System", has been revised.

Three traceable Cesium-137 sources of different activity levels are now used. In

addition, a traceable Cobalt 60 source is employed in this procedure. The inspector had no further questions.

(Closed) IFI 81-07-41 - This item concerned the lack of quality assurance audits of health physics performance during shipping of radioactive materials. A system for audits is now in use. The inspector reviewed audits performed November 6,13, and 20, and December 11, 18, 23, and 30, 1981. A tracking system is in use for noted deficiencies.

The inspector had no further questions.

(Closed) IFI 81-10-02 - This item concerned the recommendation that the licensee reevaluate the use of multiple badging for steam generator work. A thorough evaluation was done by the licensee and procedure changes resulted from this study. All personnel working in steam generators now wear moni-toring devices on the head, the chest, the gonad area, and on both wrists.

The inspector had no further questions.

(Closed) IFI 81-10-05 - This item concerned the lack of documentation for job specific training, particularly in the area of steam generator work.

Three specific courses are now given for different aspects of the work. All training is now documented. The inspector had no further questions.

6.

Procedures The inspector reviewed'the following procedures:

HP-12, Revision 10, titled " Steam Generator Inspection and Maintenance" HP-7, Revision 11, titled " Radioactive Work Permits" HP-9.1 titled " Dosimetry Issue" HP-9.2 titled " Exposure Limits" HP-9.3 titled " Exposure Tracking" HP-9.4 titled Allowed Dose and Training Prerequisites" HP-9.5 titled " Termination of Radiation Workers" HP-9.6 titled " Dosimetry and Incident Investigations" HP-9.7 titled " Monthly TLD Badge Changeout" HP-9.8 titled " Records Maintenance" The content of these revised procedures were examined in detail by the inspector.

In addition, these procedures were discussed with various licensee representatives.

The revisions appeared to be excellent and all questions asked by the inspector were answered in a candid and forthright manner. Many of these changes made it possible to close outstanding items from previous inspections. The inspector had no further questions.

7.

Training a.

Three different courses are presented to various personnel involved in

.

steam generate work. The first course is mandatory for all personnel,

involved in any way.

This course is titled " Steam Generator Worker Training".

The second course is specifically for health physics personnel covering steam generator work. The third course is mandato y for all personnel making entries into the steam generators and is given

,

at the steam generator mockup.

The inspector was required to attend the course mandatory for steam generator workers.

The duration of this course was approximately two hours and a test was given at the conclusion. Emphasis was placed on procedures, dosimetry, exposure levels, and the fact that health physics personnel were in charge during this work.

The instructors appeared to do a good job in making the course content understandable to both experienced and non-experienced personnel.

The inspector observed training in progress at the steam-generator mockup.

Two instructors were involved in this training, one an experienced jumper, and the other a fully qualified health physics technician. This training appeared to be extensive and involved all aspects of working in a steam generator.

The inspector was unable to attend the specific course given to health physics personnel involved in steam generator work coverage.

This course was discussed with the instructor. The duration of this course

is approximately three hours and based on the examination given at the end of the course, the inspector concluded that this course appears to i

be comprehensive and adequate.

8.

Tour of Containment The inspector entered containment during the evening of March 9, 1982, and

,

observed the initial entries (jumps) of both the hot and cold legs of "B" steam generator. The health physics control of this job was excellent and this work proceeded very smoothly. Procedure changes, noted earlier in this report, were in effect and appeared to be followed precisely. Good ALARA practice was employed in that personnel were not allowed to enter the platform area until the work was actually ready to proceed. In addition, the health physics technicians were very cognizant of the lowest background area on the platform and required personnel to remain at this area when not

involved in a specific task.
