Dear Mr. Kingsley:
This will acknowledge receipt of your letter dated March 2,1998, in response to our i
letter dated February 5,1998, submitting a Notice of Violation associated with the failure of I
Radiation Protection to issue faceshields used in contaminated areas. We have reviewed your corrective actions and have no further questions at this time. These corrective actions will be examined during future inspections.

J. A. Grobe to O. letter dated February 5,1998 Enclosed is Commonwealth Edison's (Comed's) response to the Notice of Violation (NOV) transmitted with the referenced letter. The report contained one Level IV violation:
Violation 98-002-01 cited a failure of Radiation Protection to issue faceshields used in contaminated areas.
This letter contains the following new commitment:
For Violation 98-002-01:
1. The Comed Radiation Protection Handbook, Revision 1, will be implemented by March 31,1998. (NTS 254-100-98-00201.01, due 03/31/98)

NOTICE OF VIOLATION (50-254(265)/98002-01)
Technical Specification 6.11 requires that procedures for personal radiation protection be prepared consistent with the requirements of 10 CFR Part 20 and be approved, maintained and adhered to for all operations involving personal radiation exposure.
Quad Cities Radiological Procedure No. 5310-04, Revision 4, " Issuance & Maintenance of Faceshields," Step B.1, requires that faceshields used for protection from radiation or used in contaminated areas are issued by the Radiation Protection department.

Contrary to the above, on January 12,1998, faceshields were identified in the Unit 2 Decon Shop, a posted contaminated area which had not been issued by the Radiation Protection department.
This is a Severity Level IV violation (Supplement IV).
REASON FOR TIIE VIOLATION:
Comed accepts this violation.
The reason for this non-conformance of faceshields in a posted contaminated area, was lack of knowledge. The maintenance workers were not aware of the procedure requirement for faceshields used for non-radiological use in a contaminated areas.
COR.RECTIVE STEPS TAKEN AND RESULTS ACHIEVED The faceshields were removed from the contaminated area. A meeting was held on January 22, 1998 with the Mechanical Maintenance Department, which is the primary user of faceshields on site, to discuss the violation and the required controls for faceshields.
CORRECTIVE STEPS TAKEN TO AVOID FURTliER VIOLATIONS The requirements for the issuance of faceshields has been clarified in the Comed Radworker Handbook, Revision 1. Currently, all faceshields for both radiological and safety are now issued by Maintenance. The handbook also states that faceshields shall not be left in the contaminated areas unattended. The Comed Radiation Protection Handbook, Revision 1, was presented to supervisors on February 27,1998 and will be presented to site radworkers by March 31,1998.
