ILLINOIS 60137 OCT 2 2 1980 Thank you for your letter of October 6, 1980, informing us of steps you have taken to correct the appraisal findings and the noncompliance items identified in our letter of September 12, 198 In general, your proposed corrective actions for the Significant Appraisal Findings represent a positive approach to resolving these problems, thereby strengthening your overall health physics progra We will evaluate your progress in these areas during subsequent inspection However, your responses to two findings may not entirely satisfy our concern.

Our concern with HRA access (Finding 3) is that your administrative procedure may not ensure positive control over each entry owing to the availability of a large number of keys from various source For this method to be acceptable, your continuing audits should consider whether entry and exit notifications are proper and timel With regard to operator/foreman entries, it is not sufficient merely to tell the operators in training to review*the latest survey inform-tio Management should determine that it is, in fact, being done, that the information is readily available, and that it is up-to-date and accurat Furthermore, given that most such entries are made alone, self-reading dosimeters should be a requirement, not an optio In addition, where areas of abnormally high (e.g., above one R/hr) exposure rate are involved, all entries should be made with a dose rate instrumen. ~--- --~Li

OCT 2 2 1980 We found (Finding 6) during the appraisal that ARM records were often illegible despite the requirement for operator revie The problem may be inadequate recorder maintenance, too many points per chart, or bot A weekly, or even daily, review of the record for trends is a good practice but does not appear to directly address our concern over legibility of the recor We ask that the concerns expressed above be considered in your corrective actions on these two matter Your corrective actions for the noncompliances will also be examined during a future inspectio cc w/ltr dtd 10/6/80:
