  • REASON FOR INVESTIGATION On September 5, 1980, Region III was advised that on August 8, 1980, an NRC resident inspector had observed two reactor operators apparently asleep while on duty in the control roo SUMMARY OF FACTS At about 6:00 a.m., on August 8, 1980, the NRC Senior Resident Inspector at Dresden Nuclear Power Station walked through the control room and observed the Reactor Operator (NSO) at the center desk and the NSO assigned to Unit 3 apparently aslee The Inspector stated that he had observed the NSO at the center desk with his arm outstretched on the desk with his head resting on his ar He also stated the Unit 3 operator was observed with his arms folded on his desk with his head resting on his arm He said that, although he did not speak to or approach anyone present in the control room, he was convinced the two operators were aslee The Inspector proceeded to the Shift Engineer's office and informed him of his observatio The Shift Engineer made a telephone call to the center desk and instructed the NSO to assure that everyone was awake in the control roo The NSO who was at the center desk stated that he was reading a book with his head resting on his upper arm, a position he frequently assumes when readin He stated he may have closed his eyes for a few seconds but denied that he was aslee He also stated that a few minutes earlier he had recorded some instrument reading The Unit 3 NSO stated he was reading a book with his arms folded around the book, holding it open, and his head was bent dow He stated that he may have closed his eyes for a few seconds, but at no time did he loose awareness of his surrounding He also stated he had prepared a turnover sheet a few minutes earlie Both operators indicated that they were fighting slee The other two NSO's present in the control room, the Unit 1 and Unit 2 operators, stated they did not see the Inspector the first time he passed through the control room, and both stated they did not observe any one asleep or apparently asleep in the control roo All of the operators indicated that on the third shift there is usually little or nothing to do, there is little traffic through the control room and each operator is essentially isolated from the other The existing conditions in the control room on this shift make it difficult to remain alert at all time One item of noncompliance was identified during this investigatio Contrary to 10 CFR 50.54(k) two of the three licensed operators assigned to monitor the operation of Units 2 and 3 appeared to be asleep, such that they were not fully attentive nor were they able to monitor reactor condition *

  • Denotes those present during the exit meeting on September 10, 1980. Introduction On September 5, 1980, Region III was advised that on August 8, 1980, the Senior Resident Inspector at the Dresden Nuclear Power Station had observed two reactor operators asleep while on duty in the control roo.

Interview with NRC Senior Resident Inspector On September 8, 1980, J. L. Barker, NRC Senior Resident Inspector at Dresden Nuclear Power Station, was interviewed and he provided a sworn statement, a copy of which is attached to this report as Exhibit Barker advised that on August 8, 1980, which was a Friday, he arrived at the Dresden Site at about 6:00 After a brief stop at his office he proceeded directly to the control roo He entered the control room through the south door, paused for a few seconds as he reached the approximate center of the control room and then proceeded out of the control room through the west doo While in the control room, Barker stated, he observed the operator at the center desk with one arm outstretched on the desk with head resting on i He said that he did not recall seeing any reading material on the desk in front of the operato Although he did not see the operator's eyes, Barker said that he was personally convinced the operator was aslee * Barker also stated he observed the Unit 3 operator with his arms folded on his desk with his head resting on his arm Since he could not see this operator's eyes in that position, he could not say whether they were open or closed, but he 1was also personally convinced that operator was also aslee Barker stated he did not approach either of the operators and did not speak to anyon He indicated he also observed the o~rators at Units 1 and 2 and both were sitting upright and readin Barker stated a shift change occurs at 7:00 a.m. and that the turn-over from one shift to another begins at about 6:50 a.m. and is completed by 7:20 He indicated that during the third shift there is little traffic through the control roo Barker stated that he proceeded from the control room directly to the Shift Engineer's offic As he entered the Shift Engineer's office he asked the Shift Engineer if he was aware that there were operators asleep in the control roo Barker said the Shift Engineer picked up the telephone before he could stop him and called the center desk of the control roo He recalled that the Shift Engineer said on the telephone something to the effect, "If anyone's asleep, hold reveille".

Barker indicated he had intended to have the Shift Engineer accompany him to the control room to observe the conditions he had observed in passing through there a few minutes earlie Barker stated it is his practice to take any problems he observes to a supervisory individual rather than to discuss problems he encounters with non-supervisory personne Barker stated he remained in the Shift Engineer's office for approx-imately a half-hour discussing other matters before he departe When he left, he passed through the control room again and at that time all personnel present were awak Barker stated that for the remainder of that day he was occupied with other matters and did not discuss the matter with the Plant Superin-tendent until Monday, August 11, 198 The Plant Superintendent, who at that time was B. Stephenson, informed him that the matter would be investigate Barker indicated it was his understanding that later the same day, or possibly the following day, the Plant Superintendent interviewed the center desk NSO in the presence of other management personnel and a union stewar He heard that during the interview the operator had denied being aslee Barker said he understood that the operator had stated he had his head down but he was looking at some papers on his des The NSO, who had been assigned to Unit 3 on August 8, 1980, was on vacation during the week of August 11, 1980, and was not interviewed by plant management until about two weeks late Barker indicated that on or about August 13, 1980, B. Stephenson was replaced as a plant superintendent by D. Scot He indicated he had talked to Scott about the problem he had encountered in the control room and indicated that he expected some corrective action to be take During a subsequent meeting the Plant Superintendent in-formed him that the Unit 1 and 2 operators had not seen Barker in the control rQom and that the Unit 3 operator has also denied being asleep when Barker was in the control room on August 8t The Plant Superintendent informed Barker a letter concerning the incident would be put in the personnel files of the two operators and that action was being taken by management to provide relief for the operators at least once during each shif In addition, the Plant Superintendent indicated that a memorandum to all operators would be issued on the subject of sleeping while on dut Barker stated that, since he did not have any witnesses to confirm his observations, he felt that there was not a sufficient basis for citing the licensee for an item of noncompliance concerning the con-dition he had observed on August 8, 198 He indicated he was also satisfied that plant management had taken corrective action concerning the matter and he had considered the matter essentially closed at that poin. Interviews with Two NRC Resident Inspectors On September 8, 1980, Thomas M. Tongue and Micheal J. Jordan, both NRC Resident Inspectors at Dresden, were interviewe Both individ-uals provided sworn statements, and copies of these statements are attached to this report as Exhibits B and C, respectivel Both individuals stated they had no first hand knowledge relating to the conditions observed by Barker on August 8, 198 They indicated that Barker had informed them of what he had experienced and both indicated that they had not observed any operators asleep during any visits they had made to the control roo. Tour of Control Room On September 8, 1980, a tour was made of the Dresden Control Room accompanied by Barker, the NRC Senior Resident Inspector, who de-scribed the path he had taken on August Attached to this report as Exhibit D is a sketch of the lay-out of the control room showing Barker's path through the control room on August 8, 198 It was noted during the time spent in the control room that there is a con-stant drone or humming soun It was further observed that the movements of personnel in the control room made no sound because the room is carpete During the visit to the control room the Operators Logs for Units 2 and 3 as well as the center desk log were reviewe It was noted that on August 8, 1980, the Unit 2 Operators Log showed an entry at 0345 and the next entry was at 075 The Unit 3 Operators Log for that date showed an entry for 0402 and the next entry was at 080 The center desk log for this date showed entries at 0350 and another at 062 The next entry in the center desk log took place during the following shif. Interview with Shift Engineer On September 9, 1980, Richard C. Goodin, Shift Engineer, was inter-viewed and he provided a sworn statement, a copy of which is attached to this report as Exhibit The Shift Engineer stated that on August 8, 1980, at about 6:00 or shortly after that, the Senior Resident Inspector entered his office and said something to the effect that "Goodin, wake up your sleeping NSO's".

The Shift Engineer stated he had asked the Inspector which ones were aslee Barker responded that the two out of three were aslee The Shift Engineer said that this was clear in his recollection because there were four NSO's present in the control roo He went on to explain that there was no specific NRC requirement that there be a~ NSO on Vnit 1 since it had been shut down for several month However, for other reasons, the Unit 1 panel was being continuously manned by an NSO. . The Shift Engineer stated that the Inspector proceeded across the office and sat down at the Unit 1 foreman's desk and began to engage in conversation with the Unit 2 forema The Shift Engineer stated that the Inspector had an opportunity to request that he accompany him to the control room if he had desired that he do s He indicated that he was not altogether sure that the inspector was not joking when he entered his office and said to wake-up the sleeping NSO' The Shift Engineer stated that after the Inspector had sat down, he, the Shift Engineer, called the center desk of the cpntrol room on the telephon He stated that the telephone was answered after a maximum of two ring He informed the operator who answered the telephone that an NRC inspector had been in the control room and had accused the personnel there of being aslee He told the operator that if anyone was asleep in the control room that they should "hold reveille."

The Shift Engineer went on to state that it was his recollection that the NSO who had answered the telephone had denied that anyone was asleep in the control room during their telephone conversatio Although the Inspector remained in the Shift Engineer's office for .several minutes, there was no further conversation regarding the conditions that the Inspector.had observed in the control roo The Shift Engineer stated that he had been in the control room about - 6 -

. ~' 4:30 a.m. that morning and possibly had visited the control room again prior to the Inspector's visi He stated he did not see anyone asleep in the control room at that time and had never seen anyone asleep at any time in the control roo He had stated he had on occasion seen an operator who was nodding or drowsy but that he had not observed any one actually aslee The Shift Engineer stated that since there was no further conversa-tion with the Inspector concerning the matter, he had forgotten about the occurrence until the subject was brought to his attention by the Plant Superintendent the following Monday, August 11, 198 The Shift Engineer stated that since August 8, 1980, the management has provided a means of relieving the operators on duty at least once during each shif He indicated that on the third shift the control room is usually quiet and there is little, if anything, going o The operators assigned to that shift are present in an atmosphere which is conducive to boredom and it is difficult for them to remain alert at all time. Interview with the Unit 3 NSO On September 9, 1980,' Lawrence H. Berg, the NSO who was assigned Unit 3 during the 11:00 p.m. to 7:00 a.m. shift on August 8, 1980, was interviewed and he provided a sworn statement, a copy of which is attached to this report as Exhibit George G. Turner, NSO and Steward, !BEW Local 1460, was present at the Unit 3 NSO's request during this intervie The Unit 3 NSO stated that a few minutes after 6:00 a.m. on August 8, 1980, he heard the phone ring at the center desk. * After the NSO at the center desk completed the telephone conversation he saw him approaching his desk and, as he approached him, he asked him what he wante The center desk NSO said that the Shift Engineer had called and said that an NRC inspector had walked through the control room and said that some operators in the control room were aslee The Unit 3 NSO requested the center desk NSO to relieve him and he went to get coffee or other refreshments for himself and the other NSO's in the control roo The Unit 3 NSO stated that at about 5:00 a.m. he had been relieved by the NSO at the center desk for a few minutes and he had gone to get coffee or cokes for the other NSO At about 5:45 a.m. to 5:50 he wrote out his turnover sheet which is for the benefit of the NSO who would relieve hi When he completed the turnover sheet, he began to read a boo He said he had his arms folded around the book, hold-ing it open, with the book on his des He indicated that his head was leaning over the desk with his head bent dow He indicated that, although he did not specifically recall it, he may have had his eyes closed for a few seconds and that he was fighting slee He stated - 7 -

that, although his eyes may have been closed for a few seconds, he at no time lost awareness of his surrounding He said he recalled having the sensation of his head bending forward with his eyes closed and then his head snapping backward and opening his eye He stated he did not see the Inspector enter the control room that morning. _He estimated he had been reading for about fifteen minutes before he heard the center desk telephone rin The Unit 3 NSO stated that he went on a one-week vacation the fol-lowing week and was interviewed by plant management upon his retur He indicated that he had attended a second meeting on about September 2, 1980, in which management stated that they r'ecognized that this was not an individual proble Management had stated that they would provide some relief for the NSOs on duty in the control roo He also indicated a report of the occurrence had been placed in his personnel fil. Interview with the Center Desk NSO On September 10, 1980, Kurt E. LaVoie, the NSO assigned to the center desk on August 8, 1980, on the 11:00 p.m. - 7:00 a.m. shift was interviewed and he provided a sworn statement, a copy of which is attached to this report as Exhibit George G. Turner, NSO and Steward, IBEW Local 1460, was present during this interview at the request of LaVoi The Center Desk NSO stated the individual assigned to the center desk is required to record hourly readings of various instrument These readings are recorded on a sheet with the hours of the day pre-printed on i The actual time that readings are taken is not recorde In order to accomplish this task it is necessary for him to leave the desk to obtain the readings from the control panels at the west wal of the control roo He indicated that this task requires about 5 to 10 minutes to complet He said it was his recollection that he had taken the 6:00 a.m. readings between 5:45 a.m. and 6:00 He stated that, after this task was completed, he began to read a boo He stated he did not see Barker enter the control roo The Center Desk NSO stated he was reading the book with his head resting on the upper portion of his right arm, a position he fre-quently assumes while reading~ He indicated that he was reading in this position when his telephone rang at approximately 6:10 He answered the telephone after the first or second rin The cal.ler was the Shift Engineer who said that an NRC inspector had been in the control room and had complained to the Shift Engineer that he had observed some operators aslee Following his conversation with the Shift Engineer, the Center Desk NSO stated he walked toward the Unit 2 NSO and observed that he was standing at the control pane He turned and walked toward the Unit 3 NSO and observed that he had his head down with a book in his han He said that he had also observed the Unit 1 NSO reading a boo The Center Desk NSO stated that, as he approached the Unit 3 NSO, the latter asked him what he wanted, and he informed him an NRC inspector was with ~he Shift Engineer and was claiming that the operators in the control room were aslee The Unit 3 NSO asked to be relieved and the Unit 3 NSO went to get coffee for the other NSOs in the control roo The Center Desk NSO stated that the Unit 3 NSO did not seem to be asleep to hi He stated that he did not see anyone asleep in the control room on that shif The Center Desk NSO said that, when the Inspector entered the control room he might have had his eyes closed, but he maintained that he was not sleepin He indicated there had been no occasions while he was on duty in the control room when he would have had his eyes closed for as much as one minut He stated that on August 8, 1980, he had telieved the Unit 3 NSO sometime between 5:00 and 5:30 a.m. so that the Unit 3 NSO could go for coffee or other refreshments for the other NSO The Center Desk NSO stated he saw the Inspector come back into the control room about 10 minutes after the phone call was received from the Shift Engineer.. He indicated the Inspector did not say anything to him but proceeded on out of the control roo The Center Desk NSO said that he had never seen any operators on duty in the control room really sound aslee He said it is very quiet in the control room during the third shift and there is little or no traffic through the control room during that shif He indicated that, when he feels drowsy, he usually gets up and walks around or talks to someon He indicated that conversation with the other NSOs, which is helpful in remaining alert, is more difficult now because they are required to remain at their desks at all time unless relieve He said that until about two years ago the NSOs had customarily spent some time around the center desk conversing with one another and this helped to relieve the boredom that is experienced during the third shif. Interview with Unit 2 NSO On September 9, 1980, Paul E. Planing, the NSO who was assigned to Unit 2 on August 8, 1980, during the 11:00 p.m. - 7:00 a.m. shift, was interviewed and provided a sworn statement, a copy of which is attached to this report as Exhibit George G. Turner, NSO and Steward, IBEW Local 1460 was present during this interview at the request of Planin The Unit 2 NSO stated he did not see the NRC Inspector in the control room the first time he entered on August He said he was also not aware of the telephone call from the Shift Engineer to the center des He stated that at about 6:15 - 6:30 a.m. the Inspector did enter the control room and came over to his desk, said hello and then left. After the Inspector departed, one of the NSOs told him that the Inspector had said he had observed some of the NSOs in the control room aslee The Unit 2 NSO stated he is of the op1n1on that he was standing at the control panel equalizing the drywell to torus differential pressure when the Inspector entered the control room the first tim He indicated that he could have been sitting at his desk reading a boo The Unit 2 NSO stated he did not notice anyone who appeared to be asleep or with his head down during that shif He made the observation that thiNSO, who was assigned to the center desk on that shift, frequently writes with his head resting on his ar He indicated he could not see the Unit 3 NSO from his desk without rolling his chair away from his d~sk. He said he does not usually look in that direction unless he hears an alarm or has something specific to say to the Unit 3 NS The Unit 2 NSO said the third shift is a period when there is little or no traffic in the control roo He indicated he did not recall seeing anyone enter the control room for about an hour before the Inspector entered at abour 6:15 a~ The Unit 2 NSO stated he has never observed anyone asleep in the control room and there have been no occasions when a telephone or an alarm had not been answered promptl He indicated he has experienced drowsiness on some occasions, has caught himself nodding or been in a condition in which he was fighting slee He indicated that, when the Unit to which he is assigned is running smoothly during the third shift, there is little for him to do and the working environment is one of boredo He indicated that this is a problem faced by all of the operators at this plant and, in his opinion, this is a common problem throughout the industr. Interview with the Unit 1 NSO On September 10, 1980, Timothy P. Meents, the NSO who was assigned to Unit 1 on the 11:00 p.m. - 7:00 a.m. shift on August 8, 1980, was interviewed and he provided a sworn statement, a copy of which is attached to this report as Exhibit George G. Turner, NSO and Steward, IBEW Local 1460, was present at the Unit 1 NSO's request during this intervie The Unit 1 NSO stated it was his recollection that at about 5:30 on August 8, 1980, the Unit 2 NSO asked him if he wanted a cok From that time, until he saw the Inspector come through on what he - 10 - unaerstands was his second visit to the control room, the Unit 1 NSO said he was readin He indic~ted he usually faces toward the center desk while he is reading but he did not recall with certainty whether he was facing that direction on August 8, 198 In any event he indicated that he can not see the individual at the center desk because there are obstructions which block his view of that positio He stated he did not see the Inspector the first time he walked _through the control.room on August 8, 198 * The Unit 1 NSO stated that he cannot see the Unit 3 NSO from his position at Unit He stated he did not see any operators who were asleep or who appeared to be asleep during that shif He indicated that he may have heard the telephone ring at the center desk when the Shift Engineer called at about 6:15 a.m~ but he does not specifically recall i He said he was not aware of the Inspector's complaint about NSOs being asleep until after he had observed the Inspector pass through the control room the second tim He indicated that, when he is assigned to Unit 1, he is permitted to move around the control room in contrast to the Unit 2 and Unit 3 NSO He indicated he was in the area of the center desk when the inspector came through the second tim He indicated that the Inspector may have said hello but he did not stop to talk with anyon The Unit 1 NSO stated he was asked by C. E. Sargent, Operating Engineer, who is a member of plant management, about what had transpired on August 8, 198 He indicated he was unable to provide any information as to whether or not the other NSOs were asleep on that shif. Interview with Operating Engineer On September 9, 1980, C. E. Sargent, Operating Engineer, was inter-viewe Sargent stated that, when the NRC Inspector brought the matter to the attention of the Plant Superintendent on August 11, 1980, arrangements were made to interview the NSO who was assigned to the center desk on August 8, 198 This interview was conducted by B. Stephenson, who was then Plant Superintendent, on August 12, 1980 with other management personnel as well a union steward presen Since the NSO, who had been assigned to Unit 3 on August 8, 1980, was away on vacation, that interview was not conducted until' August 18, 198 This interview was conducted by Sargen In each instance an Employee Report was prepared containing the information discusse A copy of the Employee Report for each individual was placed in the individual's personnel fil Copies of these Employee Reports were obtained and are attached to this report as Exhibits J and In both instances the NSOs were given a formal oral warning concerning sleeping in the control room while on dut Sargent also advised that a memorandum, dated August 22, 1980, was sent to all Operating Shift Supervisors and Nuclear Station Operators on the subject of operating personnel found sleeping in the control roo A copy of this memorandum was obtained and is attached to this report as Exhibit Sargent stated this memorandum also referenced an earlier memorandum concerning the subject of employees found sleeping which was dated November 8, 197 A copy of this earlier memorandum was also obtained and is attached to this report at Exhibit Sargent stated the August 22, 1980 memorandum provides for the utilization of the Unit 1 NSO to provide relief for the other NSO's assigned to the control room as a means of assuring that the NSOs remain alert at all times in the control roo Sargent stated that corporate management had been made aware of the problem and the actions that had been taken by plant management regarding i. Exit Meeting On September 10, 1980, an exit meeting was held which was attended by those individuals identified by an asterisk (*) in the Persons Contacted Section of this repor Licensee management was advised that this matter was considered to be a serious one by the NRC and that the NRC was not only concerned about NSOs being asleep but also that they remain alert at all times while on dut Attachments: Exhibits A through M - 12 -

Nuclear Regulatory Commissio I make this statement freely with no threats . or promises of reward having been made to m Mr. Phillip is writing this statement for me at my reques I have been employed with the Region III office of the.NRG since October 197 I worked as an inspector in the Reactor Operations Support.Section until March 197*8 at which time I was designa.ted as the Project Inspector for the Dresden Nuclear Power Statio I have been at Dresden as a Resident Inspector since July 1978 and was designated the Senior Resident,)nspector in October 197 "* On August 8, 1980 I arrived at the station at about 6:10 a.m. and arrived in the control room a few minutes late I entered the control room through the south doo.r and, as I approached the center desk, I observed the operator at the center desk with his left arm outstretched on the desk with his head resting on i He was facing away from me so I did not see his eye He did not move and I did not notice any reading material in front of hi He appeared to be aslee I proceeded west and observed the Unit 3 operator had his arms folded and his head resting on them face downwar He did not move either and he appeared to be aslee I didn't speak to anyone in the con-trol room but I observed that the Unit 1 and Unit 2 operators were sitting upright and readin I exited the ~est door of the control room and proceeded directly to the Shift Engineer's offic I said to the Shift Engineer on duty "Do you realize two of the four operators in the control room are asleep?" Before I could ask him to accompany me to the control room, he picked up the telephone and called the control room, I think the center d.es He said something to the effect, "Hold reveille in the control room".

I remained in the Shift Engineer's office engaged in conversation with him for about 30 minute When I left, I went through the control room and everyone was awake at that tim I was busy on other matters the rest of that day and on Monday, August 11, 1980, I discussed the matter with the Station Superintendent, who at that time was B. Stephenso He stated he would conduct a full investigation of the matte On Tuesday, August 12, I was informed that the operator who had been at the center desk had been interviewed and he had denied he was aslee I was informed the other operator was on vacation and would be questioned in a week to 10 days later when he returned to wor I was sub-sequently informed that upon questioning, he denied he had been aslee At that time I stated to plant management that even though there was no evidence that the operators were asleep, I wanted management to implement corrective action which would insure that operators remain alert and awake in the control roo l Exhibit A Page 1 of 2

This was my first knowledge of the inciden When I returned to the Resident Inspector's office I asked Barker about the matte He informed he had observed two operators in the control room asleep one morning the previous wee He said this occurred at about 6:00 a.m. one mornin During this conversation, or possibly during a later discussion, with Barker I asked him what was going to be done about i He said he wanted to see what the licensee was going to do about it before deciding what action he would tak During a later conversation he informed me that the matter had been brought to the attention of the corporate offic He said one operator had denied that he was asleep and the other operator was on vacatio He said he didn't think he had a firm basis for citing the licensee because the one individual had denied being asleep and he, Barker, had no witnesse At a closeout meeting with station management on August 29, 1980 Barker said he was satisfied with the corrective action taken by the licensee re-garding the matter of operators being asleep in the control roo Since this was a finding of Barker's I left the question of informing the Region III office to hi I did not know whether or not the Region III office was informed until Thrusday, September 4, when a reporter made inquiries about the matte The *formal reporting period was from August 3 to September 6, so I was not surprised that the matter had not been discussed with Region II Items of noncompliance identified during a reporting period may be discussed with

Stephenson, Dated November 8, 197.7), represents the current station policy fo~ all employees except those in the Control Room with license duties that limit their mobility (N.S.O.). This exception is taken since it is recognized that severely limited mobility (assigned to a desk) for long periods of time is not conducive to the expected performance level for these employee, In order to help alleviate this problem, the following guideline will be used on a trial or interim basi ~is guideline is.subject to change upon written notice to the personnel involve Effective immediately, the Unit 1 N.S.O. may be released from his duties to provide relief £or other N.S.O.'s assigned in the Control Roo Should an N.S.O. become drowsy, he shall notify a shift supervisor that he will be leaving the Control Room and the Unit 1 N.S.O. will be covering his position. In addition, the shift supervisors are responsible to assess the condition of the N.S.O. 's on a continuing basis and to take.the necessary action to ensure all Control Room operators are alert aµd capable of performing their licensed dutie The following is a restatement of the station policy on employees found sleeping as it applies to the N.S.O.' Employees with license duties found sleeping in the Control Room may be sent home for the remainder of their shif The decision to either send ~ sleeping employee home or allow him to continue performing his duties will be made at the discretion of the shift supervisor.
