During performance of Visual Inspections required by Technical Specification 3.7 9, mechanical snubber R141-1060S was found locked between its hot and cold settings. The snubber was located on line IRl41BA-10".

V. PROBABLE CONSEQUENCES OF THE OCCURRENCE: .

The affected piping was analyzed by Sargent and Lundy with the defective snubber modeled as a rigid restraint. No adverse stresses or loads occurred Therefore, on any piping or adjacent restraints as a result of this failure.

This flashing, coupled with the large thermal movements normally experienced by the .line, caused forces and thermal movements greater than design. Snubber manufactured by Pacific Scientific.

Vll. CORRECTIVE ACTION:

The defective snubber was replaced under Work Request L24119 All other snubbers on lines 1Rl41BA,1Rl41BB and IRl41A were stroked and two similar failures found. These snubbers were replaced under Work Requests L30072 and L30073 All affected piping was analyzed as acceptable by Sargent &

Lundy. As a long term corrective action, Modification M-1-1-82-26 tracks the addition of a warming line around valve 1E51-F064.
