On May 2, 1990 at approximately F52 hours with Unit 2 defueled, while performing LaSalle Technical Staff l, Survelliance LTS-800-205, "2A Diesel Generator 24 Hour Run" (DG), Unit 2 Olvision 2 bus 242Y was l

de-energized, the 2A DG started, and energized the bus as expected. When the bus re-energized, the Control i Room "B" Emergency Makeup Ventilation Fan (0VC03CB) (VC) auto started due to a false VC High Radiation .

Initiation caused by the loss of power supplied to the Control Room Radiation Monitors. The VC system ,

functioned as designed, however the auto start of the VC Emergency Makeup Fan was not' anticipated by the procedure prior to de-energizing the bus 242Y. The fan was reset after the completion of LTS-800-205.

I The apparent cause of this event was a procedure deficiency. LTS-800-205 does not address the start of the 1

'8" VC Emergency Makeup Fan when bus 242Y is de-energized and subsequently re-energized.

LTS-800-205 and siellar procedures will be revised to address the VC "B" Emergency Makeup Fan actuation prior to energizing bus 242Y. In addition, steps will be added to reset the VC system prior to returning the system to its normal operating mode.

This event is reportable pursuant to the requirements of 10CFR50.73(a)(2)(lv) due to the actuation of an Engineered Safety Feature System.

C. APPARENT CAUSE OF EVENT

  • The apparent cause of this event was a procedure deficiency. LTS-800-205 does not address the start of the "8" VC. Emergency Makeup Fan when bus 242Y is de energized and then re-energized. -l This type of actuation may have occurred previously during this type of testing. This procedure and-siellar procedures which de-energize the Division 2 busses have been perfomed previously without ESF actuations being reported. It is possible that the associated system was not available or more Ilkely the personnel involved were aware that this would occur and therefore anticipated the' actuation or took' C action to prevent it. Recently, the station has required that these types of ESF actuations be explicitly identified in the procedure to ensure all personnel are aware that the actuation will occur.

If not'so' identified, the event is classified as reportable.

D. SAFETY ANALYSIS OF EVEN1 The VC system functioned as designed. At the time of the event, both VC Emergency Makeup systems were operable. This event did not cause the VC system to become inoperable.

E. CORRECTIVE ACTIONS LTS-800-205' will be revised to address the VC "0" Emergency' Makeup Fan starting prior to performing the steps where bus 242Y is de-energized. In addition, steps will be added to reset the VC system logic prior to returning the system to its normal operating mode. These same changes will also be incorporated into LTS-800-105 "1A_DG 24 Hour Run", LTS-500-110 " Unit 1 Division 2 ECCS Response Time Test", and LTS-500-210 " Unit 2 Division 2 ECCS Response Time Test." These surveillances will also start the VC "A" and/or "B" Emergency Makeup Fans by de-energizing and re-energizing their respective bus (142Y and/or 242Y). Action item Record (AIR) 374-200-90 4 330), has been initiated to track completion of these procedure revisions. AIR 314-200-90-03302 has been initiated to review operating procedures and abnormalities associated with de-energizing ESF busses to determine if any changes are required. -
