APPENDlX A MOTICE OF VlOLATION

. Northeast Nuclear Energy Company Docket No. 50-336 Millstone Nuclear Power Station Unit 2 License No. DPR-65 As a result of the inspection conducted on July 6-24,1992, and in accordance with the

" General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1992), the following violation was identified:

10 CFR Part 50, Appendix II, Criteria til and V, require that activities affecting quality shall be prescribed by documented instructions which correctly translate the plant design. These activities shall be accomplished in accordance with these instructions.

The implementation plan of Plant Design Change Record (PDCR) No. 2 123-91, required that vital inverters No. 2 and 6 he replaced simultaneously, and vital inverter No. 4 thereafter.

Contrary to the above, on July 6,1992, the implementation plan of PDCR No. 2-123-91 was not translated correctly into work order M2-92-06984. Consequently, vital inverters No. 2 and No. 4 and associated static switches were removed from service simultaneously, resulting in a loss of normal power to the plant.

This is a Severity lxvel IV Violation (Supplement 1).

Pursuant to the provisions of 10 CFR 2.201, Northeast Nuclear Energy Company is hereby required to submit a written statement or explanation to the U.S. Nuclear Regulatory Commission, A1TN: Document Control Desk, Washington, DC 20555 with a copy to the Regional Administrator, Region I, and if applicable, a copy to the NRC Resident inspector within 30 days of the date of the letter transmitting this Notice of Violation (Notice). This reply should be clearly marked as a " Reply to a Notice of Violation" and should include for each violation: (1) the reason for the violation, or, if contested, the basis for disputing the violation, (2) the corrective steps that have been taken and the results achieved, (3) the corrective steps that will be taken to avoid further violations, and (4) the date when full con.pliance will be achieved. Where good cause is shown, consideration will be given to extending this response time.
