RIVER BEND STATION - NRC SECURITY BASELINE INSPECTION REPORT 05000458/2016404

Dear Mr. Maguire:

On October 13, 2016, the U.S. Nuclear Regulatory Commission (NRC) completed a security inspection at your River Bend Station. The NRC inspectors discussed the results of this inspection with Mr. C. Rich, General Manager, Plant Operations, and other members of your staff. The results of this inspection are documented in the enclosed report.

The NRC inspectors documented five findings of very low security significance (Green) in this report. All of these findings involved violations of NRC requirements. The NRC is treating these violations as non-cited violations (NCVs) consistent with Section 2.3.2.a of the Enforcement Policy.

Further, inspectors documented a licensee-identified violation which was determined to be of very low security significance in this report. The NRC is treating this violation as a non-cited violation consistent with Section 2.3.2.a of the Enforcement Policy.

If you contest the violations or significance of these NCVs, you should provide a response within 30 days of the date of this inspection report, with the basis for your denial, to the U.S. Nuclear Regulatory Commission, ATTN: Document Control Desk, Washington, DC 20555-0001; with copies to the Regional Administrator, Region IV; the Director, Office of Enforcement; the Director, Office of Nuclear Security and Incident Response; and the NRC resident inspectors at the River Bend Station.

If you disagree with a cross-cutting aspect assignment in this report, you should provide a response within 30 days of the date of this inspection report, with the basis for your disagreement, to the U.S. Nuclear Regulatory Commission, ATTN: Document Control Desk, Washington, DC 20555-001; with copies to the Regional Administrator, Region IV and the NRC Resident Inspector at River Bend Station.

Five cross-cutting aspects were assigned to findings in the areas of Human Performance and Problem Identification and Resolution. Specifically: (1) River Bend Station failed to ensure that procedures were available and adequate to support nuclear safety [H.1]; (2) the licensee failed to identify issues completely, accurately, and in a timely manner in accordance with the corrective action program [P.1]; (3) the licensees work groups failed to communicate and coordinate their activities within and across organizational boundaries to ensure nuclear safety is maintained [H.4]; (4) the licensee failed to routinely conduct self-critical and objective assessments of its programs and practices [P.6]; and (5) individuals did not follow processes, procedures, and work instructions [H.8].

This letter will be made available for public inspection and copying at http://www.nrc.gov/reading-rm/adams.html and at the NRC Public Document Room in accordance with 10 CFR 2.390, Public Inspections, Exemptions, and Request for Withholding.

However, the enclosed report contains Security-Related Information, so the enclosed report will not be made publically available in accordance with 10 CFR 2.390(d)(1). If you choose to provide a response that contains Security-Related Information, please mark your entire response Security-Related Information - Withhold from Public Disclosure under 10 CFR 2.390 in accordance with 10 CFR 2.390(d)(1) and follow the instructions for withholding in 10 CFR 2.390(b)(1). The NRC is waiving the affidavit requirements for your response in accordance with 10 CFR 2.390(b)(1)(ii).
