Appendix A NOTICE OF VIOLATION Atlantic Richfield Company License No. 12-00140-04 Based on the inspection conducted on October 10 and 14, 1980,it appears that certain ments, of below.

as noted your activities were in noncompliance with NRC require-4 4 are deficiencies. Items 1 and 2 are infractions, and items 3 and 1.

License Condition No. 18 of Byproduct Material License No. 12-00140-04, states, in part, the licensee shall possess and use licensed material in accordance with statements, representations, and procedures con-tained in application dated June 24, 1977.

Item No. 11 of this ap-plication month specifies survey instruments will be calibrated on a six frequency.

Contrary to this requirement, on the dates of this inspection, survey instruments had not been calibrated on a six month frequency prior to February, 1980.

License Condition No.18 of Byproduct Material License No. 12-00140-04, states, in part, the licensee shall possess and use licensed material in accordance with statements, representations, and procedures con -

tained in application dated June 24, 1977.

Item 1 in a letter dated February 7,1972, attached to this application, specifies that a record is to be made of (a) external radiation and (b) amount of removable contamination on shipping container surfaces for all incoming ship-ments without exception, either received at Harvey or in the field.

Contrary to this requirement, from June 1977, to the dates of this inspection, external radiation and removable contamination measure-

- ments on all incoming radioactive shipping containers, without exception, had not been made as specified.

The " Records Management j Program" paragraph in the attachment to this application specifies that an inventory of all radioactive isotopes is made annually and maintained.

Americium-241 and strontium-90 sealed cluded in the inventory. sources, and a nickel-63 foil were not in-4.

Paragraph 10 CFR 20.401(a) Title 10 of the Code of Federal Regu-lations, Part 20, states, in part, each licensee shall maintain records of radiation' exposure of all personnel as required under 10 CFR 20.202.

Such records shall be kept on Foru NRC-5 or on clear and legible records containing all the information required by Form NRC-5. ,

Contrary to this requirement, on the dates of this inspection, all of the information licensee's records.required by Form NRC-5 was not included in the The exposure results processed by Landauer, Jr. and Co. did not include names, social security numbers, sex and birthdates as delineated by Form NRC-5.
