Subsequent totheNovember17condition, researchintothesystemdesignandoperating historyshowedthat,priorto1988,theneutronfluxhighrangetripcircuitry wasnotroutinely placedinthetrippedcondition duringchanneldefeats.-

Theunderlying causewastechnical inadequacies inprocedures anddesigndeficiency, inthatInstrument andControlprocedures utilizedforchanneldefeatdidnotaddresstheneedforneutronfluxlowrangetripcircuitry tobeinthetrippedcondition, andtheoriginalplantdesignrequiredliftingleadstoplacetheneutronfluxhighrangetripcircuitry inthetrippedcondition.

Whilethiscondition isnotreportable fortheNovember17,1997,condition, thiscondition wouldhaveoccurredduringphysicstestingperformed inthepast,andwasnotrecognized atthosetimes.Atthosetimes,theplantwouldhavebeeninacondition prohibited bytheTechnical Specifications.

RCStemperature wasbeingincreased from350degreesFtogreaterthan540degreesFandpressurizer (PRZR)pressurewasbeingincreased fromlessthan1000PSIGto2235PSIG(normalconditions forMode2).Instrument andControl(ISC)personnel haddefeatedonenuclearinstrumentation system(NIS)powerrange(PR)channel(channelN-44)toallowitsdetectors tobeconnected toareactivity computerduringreactorstartupandCycle27physicstesting.II.DESCRIPTION OFEVENT:A.DATESANDAPPROXIMATE TIMESOFMAJOROCCURRENCES:

June9,1996(andothertimes):Physicstestingduringpreviousplantoutageswithneutronfluxtripcircuitry notinthetrippedcondition.

Estimated eventdates.November17,1997,1152EST:Discovery dateandtime.EVENT:OnNovember17,1997,atapproximately 1152EST,afterthedefeatofNISPRchannelN-44,aMainControlBoardwalkdownwasconducted bythereactorengineer.

Duringthiswalkdown, herecognized thattheNC44Pbistablelight,forneutronfluxlowrangetrip(NFLRT)circuitry, wasnotilluminated.

Thus,itwasnotinthetrippedcondition.

Aninvestigation wasconducted anditwasconfirmed thattheNFLRTcircuitry forchannelN-44wasnotplacedinthetrippedcondition, asrequiredbytheGinnaStationImprovedTechnical Specifications (ITS),LimitingCondition forOperation (LCO)REQUIREDACTION3.3.1.D.1 (whichisapplicable foronlyModes1and2).SincetheplantwasinMode3atthetimeofdiscovery, andtheNFLRTfunctionisnotrequiredtobeoperableinMode3,theplantwasincompliance withtheITS.Atestcurrentsignalwassimulated intheN-44channeltoplacethechannel's NFLRTcircuitry inthetrippedcondition.

Thisensuredcompliance withITSLCO3.3.1forsubsequent entryintoMode2,wherethisLCOwasapplicable.

Investigation ofthisevent,andresearchintothesystemdesignandoperating historyshowed:~TheIRCprocedure usedforthedefeatofNISPRchannelN-44wasspecified asapplicable inallplantmodes.Theprocedure wasdeficient, sinceitdidnotensurethattheNFLRTcircuitry wasplacedinatrippedcondition whenintheITSmodesofapplicability (Mode2andwheninMode1withthermalpower(6%)asspecified inITSTable3.3.1-1,Function2.b.NRCFORM366A(4-95)

However,aspartoftheinvestigation andresearchforthisLER,itwasdiscovered thatpriorto1988,therewereinstances whenitispresumedthattheNFHRTcircuitry hadnotbeenplacedinatrippedcondition byapplicable IRCprocedures.

Thus,frominitialplantstartupuntil1988,theplantwouldnothavebeenincompliance withtheoriginalGinnaStationTechnical Specifications atthosetimeswhenanNISPRchannelwouldhavebeenrequiredtobedefeatedwithappropriate neutronfluxtripcircuitry inthetrippedcondition.

AsperTechnical Specifications ineffectatthosetimes,theNISPRNFHRTisrequiredtobeoperableforall4channels"whenRCCAiswithdrawn".

Notethat"RCCA"standsfor"rodclustercontrolassembly".

Anytime(including physicstesting)priorto1988whenaNISPRchannelwasremovedfromserviceutilizing IRCprocedures (whenRCCAwaswithdrawn),

itisnowpresumedthatthoseactionsdidnotcomplywithTechnical Specifications requirements.

Notethatfrom1988to1990,temporary procedure changesensuredthattheNFHRTcircuitry wastrippedbyliftingtwoleads.Aftercompletion ofEWR4862(from1990tothepresent),

theNFHRTcircuitry isnowtrippedbyuseofatwo-position OT-2switch.C.INOPERABLE STRUCTURES, COMPONENTS, ORSYSTEMSTHATCONTRIBUTED TOTHEEVENT:NoneD.OTHERSYSTEMSORSECONDARY FUNCTIONS AFFECTED:

NoneE.METHODOFDISCOVERY:

Duringtheresearchconducted byplantstaff,itbecameapparentthateventhoughthecurrentITSwerecompliedwithduringthisevolution in1997,inthepasttheplantwouldhavebeeninacondition prohibited byTechnical Specifications atspecifictimes.F.OPERATORACTION'ince theplantwasincompliance withITSatthetimeofdiscovery, nooperatoractionswererequired.

FortheNISPRNFHRTcircuitry, thisalsocouldhaveoccurredifanyNISPRchannelhadbeenremovedfromserviceutilizing I&Cprocedures "whenRCCAiswithdrawn" frominitialplantstartupuntil1988.B.INTERMEDIATE CAUSE:Theintermediate causeofnotplacingNISPRneutronfluxtripcircuitry intoatrippedcondition whenrequiredisduetotechnical inaccuracies inwrittenprocedures, inthatthel&Cprocedures utilizedtodefeataNISPRchanneldidnotensureneutronfluxtripcircuitry wasinatrippedcondition, andprocedures usedtodocumenttheinoperability ofNISPRchannelsdidnotensurecompliance withITSrequirements.

ROOTCAUSE:Underlying causesforthetechnical inaccuracies inprocedures include:Interface Design,inthattheuniqueness oftheoriginaldesignforplacingneutronfluxtripcircuitry intoatrippedcondition wasnotemphasized.

Change-related trainingwasnotadequate, andprocedures werenotrevisedatthattime.DesignConfiguration andAnalysis, inthattheoriginalplantdesignwasdeficient.

Theassociated switchesweredesignedforreactortriplogictesting,andnotforchanneldefeat.Priorto1990,properdefeatrequiredthatleadsbeliftedtoensurethePRNFHRTcircuitry wasplacedinthetrippedcondition.

Training, inthattherewasnotsufficient understanding oftheneedtoplaceaffectedtripcircuitry intoatrippedcondition whendefeating achannel,andinsufficient awareness ofavailable methodsfortrippingandverifying trippingwasaccomplished.

IV.ANALYSISOFEVENT:Thiseventisreportable inaccordance with10CFR50.73,LicenseeEventReportSystem,item(a)(2)(i)(B),whichrequiresareportof,"Anyoperation orcondition prohibited bytheplant'sTechnical Specifications".

Itispostulated thatITSLCOREQUIREDACTION3.3.1.D.1 wasnotperformed duringthescheduled physicstestingofJune9,1996.Priortotheimplementation oftheITSonFebruary24,1996,itispostulated thattheoriginalTechnical Specifications, Section3.5.1.1,werenotproperlyperformed duringpreviousphysicstestingandatvariousothertimesfrominitialplantstartupuntil1996.Anassessment wasperformed considering boththesafetyconsequences andimplications ofthiseventwiththefollowing resultsandconclusions:

Therewerenooperational orsafetyconsequences orimplications attributed tonotplacingaNISPRneutronfluxtripcircuitry intoatrippedcondition because:Asdescribed intheITSbasis,thePowerRangeNeutronFluxTripFunctionensuresthatprotection isprovidedagainstanuncontrolled RCCAbankrodwithdrawal accident.

Therefore, theactuation logicmustbeabletowithstand aninputfailuretothecontrolsystem,whichmaythenrequiretheprotection functionactuation, andasinglefailureintheotherchannelsproviding theprotection functionactuation.

Asdescribed intheITSbasis,thePowerRangeNeutronFlux-LowTripFunctionensuresthatprotection isprovidedagainstapositivereactivity excursion fromlowpowerorsubcritical conditions.

Therefore, anassumedsinglefailurewouldnotpreventthistripfromactuating asdesigned.

Theintermediate range(IR)channelreactortrip(whichisabackuptothePRNFLRT)wasunaffected bythiscondition andwouldalsohavebeenavailable tomitigateareactivity transient atlowpower(1/2channels).

ForinitialstartupsitisthestandardpracticethattheNISPRNFHRTsetpointbeloweredfromitsnormalsetpointof108%to50%power.Thisreduction insetpointprovidesanadditional backuptotheNFLRT.NotetheNFHRTisproperlydefeatedbyCPI-AXIAL-N44.

Basedontheabove,itcanbeconcluded thatthepublic'shealthandsafetywasassuredatalltimes.V.CORRECTIVE ACTION:A.ACTIONTAKENTORETURNAFFECTEDSYSTEMSTOPRE-EVENT NORMALSTATUS:Nonerequired.

B.ACTIONTAKENORPLANNEDTOPREVENTRECURRENCE:

~EWR4862wasimplemented, whichresolvedthedesigndeficiency fortheNISPRNFHRTcircuitry.

~Procedures CPI-AXIAL-N-41/42/43/44 andCPI-BISTABLE-N41/42/43/44 willbereviewed, andrevisedasnecessary, toensureproperdefeatofneutronfluxtripcircuitry.

Notethatoperations procedures willnotbechanged,sincetheyaccomplish properdefeat,including placingrequiredtripcircuitry inthetrippedcondition.

~Thedesignandoperation ofthebarrelswitch,whichisavailable toplaceaPRNFLRTcircuitinatrippedcondition, willbethesubjectoffuturetrainingforIRC,operations, andotherappropriate personnel.

ThisissuewasnotreportedtotheNRCwheninitially discovered in1988.In1998,administrative processes foridentifying andreviewing adverseconditions weredeficient.

Thiswasrecognized, andtheACTIONreporting processwasimplemented.

TheACTIONprocessensuresthatsuchissuesareproperlyevaluated forNRCreportability.
