NRCFORM36BA(91998)ILICENSEEEVENTREPORT(LER)TEXTCONTINUATIONU.S.NUCLEARREGULATORYCOMMISSIONFACILITYNAMEI1)R.E.GinnaNuclearPowerPlantDOCKET(2)05000244LERNUMBERIe)y~ASEOUENIIALREVI9IONNUNREANUMBER1999-S01-00PAGEI3)2OF3TEXT(ifmorespaceisrequired,useaddidonalcopiesofNRCForm366A/I17)Pre-EventPlantConditionsTheunitwasinMode1,atnormaloperationsatapproximately100losteady-statereactorpower.DescriptionofEventOnApril29,1999,theGinnaStationAccessAuthorizationofficereceivedtherequestedmilitaryrecordsforanindividualwhohadbeengrantedtemporaryunescortedaccessonFebruary18,1999.Afterreviewofthisdocumentation,itwasdeterminedthattheindividual'strustworthinessandreliabilitycouldbecalledintoquestion.Unescortedaccesswassuspendedimmediately(onApril29)pendingcompletionofaninvestigation.Uponcompletionofthisinvestigation,unescortedaccesswasrevokedonJune17,1999.III.EventSummaryWhilereviewingtheDepartmentofDefenseformDD-214suppliedbythisindividualduringin-processingforunescortedaccesstoGinnaStationinFebruary1999,itwasnotedthathehadanhonorabledischargefromtheU.S.Navy.Thenarrativereasonforseparationwas"PersonalityDisorder".AspartofthebackgroundinvestigationprocesstheindividualwasrequiredtohaveaclinicalinterviewwiththeRochesterGasandElectric(RG&E)staffpsychologist.DuringthisinterviewtheinformationregardinghisNavydischargewasdiscussed,alongwithotherpersonalinformation.Basedontheinformationobtainedatthetime,thepsychologistrecommendedtheindividualforunescortedaccess.Therestoftheindividual'sbackgroundinvestigationdidnotrevealanyotherinformationthatwouldindicatetheindividualtobeuntrustworthyorunreliable.PerNRCRegulatoryGuide5.66,itisnotrequiredtodoanyadditionalinvestigationintoamilitarydischargethatishonorable.InformationwasreceivedfromtheNationalPersonnelRecordsCenter(NPRC)onApril29,1999.AttachedtotheDD-214receivedonApril29wasadditionalinformationpertainingtotheindividual'spsychologicalevaluationconductedwhilehewasintheNavy.TheRG&Estaffpsychologistwasprovidedwiththisadditionalinformation.HisrecommendationwastoattempttocontacttheNavytoobtainmoredetailedinformation.Theindividual'saccessauthorizationwassuspended(onApril29),pendingreceiptofthisinformation.WhenRG&EwasunabletoobtainadditionalinformationfromtheNavy,the-psychologistrecommendedacompleteindependentpsychologicalevaluation.OnJune3andJune10,1999,theindividualwasevaluatedandtestedbyanindependentpsychologist.ThisreportwasthenforwardedtotheRG&Estaffpsychologist.UponreviewofthereportonJune17,1999,itwasrecommendedthattheindividual'sunescortedaccessauthorizationberevoked.
