I.IntroductionandTestSummarTheReactorPressureVesseIInternalsVibrationTestwassuccessfullycom-pletedattheSusquehannaSteamElectricStationonOctober10,1980.Thepur-:poseofthistestwastorevealloosepartsorassemblyerrorswhichcouldcausevibrationofreactorinternals.ThiswasaccomplishedbyoperatingtheRecircu-lationsystematratedvolumetriccoreflowandperforminganinspectionprogramtodeterminetheeffectsofthevibrationinduced.TheVibrationTestconsistedofapreinspection,flowtesting,andapost-inspectionwhicharesummarizedinsectionsII,V,andVIrespectively.Followingthepreinspectionandprecedingtheflowtest,datawastakenforthecompletionoftheReactorRecirculationsystemflowtesting(TP2.20).TP2.20willbeat-tachedtotheReactorRecirculationPreoperationalTestP64.1.Duringtheperfor-manceofthisflowtestingthetemporarymanwayaccesscoverlocatedbelowthe"B"Recirculationsuctionpipewasdislodgedfromtheshroudsupportplateandbecamewedgedinthe"B"loopsuctionpipeimmediatelybeforethesuctionvalve.AdetailedaccountofthisproblemisgiveninsectionIII.Atthispoint,RecirculationFlowTestingwasstoppedandtheReactorpressurevesselhydrowasperformed.Whenthehydrotestwassuccessfullycompleted,thereactorvesselwasdrainedandtheRPVheadandshroudheadwereremovedtofacil-itateaninspectionofthedamagecausedbythemanwaycover.ThemanwayaccesscoverandattachinghardwarewereretrievedfromthevesselandareinspectionwasperformedassummarizedinsectionIVofthisreport.Thevesselwasthenfilledandtheheadwastensioned.The'lowTestwascompletedandthepost-inspectionwasperformed.4.Acopyoftheprocedurecontainingdatataken,graphsandphotosisincludedasAttachmentIalongwithNon-ConformanceReportsissuedduringthistest.(Attachment2).II.Pre-InsectionApreflowtestinspectionwasconductedonthereactorvesselinternalstoestablishtheconditionofeachcomponentexamined.Theinspectionpointsaredefinedinsection'7.1oftheattachedprocedure.Theinspectionteamconsistedoftwoengineers,oneQCInspectorandaphotographer.AseparateinspectionwasperformedbyGeneralElectricI&SE.Droplights,flashlights,inspectionmirrorsanda35mmcamerawereusedduringtheinspection.Allequipmenttakenintothevesselwasattachedtothepersonusingittomaintainvesselcleanliness.Are-marksheet(AppendixJ)ofeachareainspectedalongwithphotographsareincludedaspartofAttachment1.Sincetheguidetubeswerenotinstalled,itwasrecommendedbytheGeneralElectricOperationsManagerthatacompleteinspectionofallincoreguidetubestabilizersbeperformed.OnlytheperipheralstabilizerswererequiredtobeinspectedbytheGeneralElectricPreoperationalSpecification22A227IAX/Rev'.0Twomissingtackweldsandoneporousweldwasobservedon'thisinspection.Thelocationof"theseweldsarenotedontheremarksheetforstep7.1.2oftheat-tachedprocedure.Threecrackedandtwoporousweldswereobservedonthecore(,Page2of6 Isprayspargernozzlesatlocationslistedonremarksheetforstep7.1.18.TheseweldswererepairedandreinspectedbeforethestartoftheVibrationTest.Aftercompletionofthepreflowinspection,thetemporarymanwayaccesscoverslocatedbelowtherecirculationloopsuctionlineswereinstalledontheshroudsupportplate.Theshroudheadwasinstalledandtorquedto50ft/lbs.III.ReactorRecirculationFlowTestinFollowingthepreflowinspection,thevesselwasfilledtotwofeetbelowtheflange.DatawastakenforthecompletionofReactorRecirculationFlowtesting(TP2.20).Thefollowingtestswereperformed:M-Gsetcontrollertun-ing,limiter/31&f!2runbacks,scooptubelock,cavitationsearchfor"A"and"B"pumps,instrumentcalibrationchecksandventing,andRPTtripsat30,50and80Xpumpspeeds.InordertoreachthepumpspeedsrequiredforRPTtripsandlimiterrunbacks,thevesselwaspressurizedto100psiwithinstrumentairthroughtheheadventtemporarymanifoldsupplyline.Reactor.vesselpressurewasmonitoredusingatemporarygageonthenitrogenmanifoldandonLR/PR-1R623Ainthecontrolroom.Whiletryingtoobtain80/pumpspeedtoperformaRPTtrip,cavitationwasdetectedatthevesselheadwith"A"pumpat74Xand"B"pumpat70/speed.Pumpspeedswerereducedto65Xtostopjetpumpcavitation.Itwasnotedatthistimethat"B"loopdriveflowindicationswerereadingzeroandjetpump$/10readlow.Theyreadzeroforabout20min'-utes.Thisisbelievedtobefromthetemporarymanwayaccesscoverslidingover,the"B"recirculationsuctionlineelbowtaps.AnRPTtripofbothre-circulationpumpswasperformedat50/speed.ReactorPressurewasincreasedto150psiusingNsupply.Recirculationpumpswerestartedandincreasedto50/speed.Asetofdatawastakenforjetpumpsandrecirculationpumpsdriveflow.Thisdatarevealeda20/flowreduc-tionofthe"B"recirculationloopdriveflowsandjetpumpflowsascomparedtopreviousdata.Jetpumpf/10stillreadlowalso.Atthistimeaslightnoisecouldbeheardnearthe"B"recirculationpumpsuctionvalve.Previouslynoab-normalnoiseswereheard.Thecauseoftheflowreductioncouldnotbedeterminedwiththevesselfilled.IntheinterestoftimeitwasdecidedtoperformtheReactorPressureVesselHydro.Aftersuccessfullycompletingthevesselhydro,,thereactorvesselwasdrainedtodeterminethecauseofthe"B"loopblockage.WhileremovingtheRPVhead,four"0"ringretainingclipsfellintothevessel.Usingafibro-scopelookingthroughthe"B"suctionlinedecontaminationfittingitwasdis-coveredthatasteelplatewaswedgedonthevesselsi'deofthesuctionvalve.Furtherinvestigationidentifiedtheplateasthe"B"suctionsidemanwayaccesscover.PlansweremadeforretrievingthemanwaycoverandattachinghardwareandareportwasmadeonthedamagetotheReactorVesselandrecirculationsuctionpipe.Themanwaycoverandallattachinghardwarewereretrievedfromthevessel.Threeofthefour"0"ringretainingclipswerefound.ThedamagecausedtotheRPVvesselandsuctionpipewasdocumentedanddispositionedpertheNon-Confor-manceReportsincludedinthisreport(Attachment2).,IPage3of6
~~~[0 Followingtheremovalofallforeignpartsfromthevessel,areinspectiontoestablishtheconditionofvesselcomponentswasperformed.Theinspectionteamconsistedofthreeengineers,oneofwhichwasperformingthefunctionofQC.ThisreinspectionreverifiedapplicableinspectionpointsofthePre-flowInspection.Acopyofthescopeandresultsoftheinspectionisincludedasatestaddendumoftheattachedprocedure.Scratches,gouges,ducttape,andpiecesofthemanwayaccesscoverrubbergasketmaterialwerefoundthroughouttheRPVandintherecirculationpiping.Onecrackedtackweldononeofthecoresprayspargernozzleswasfound.Adentononeofthecontrolrodsandtwodislodges-peripheralfuelsupportswerefound.Thelocationofalldefectslistedareincludedintheremarksheets(AppendixJ)inthereinspectionsectionoftheattachedprocedure.Thecrackedtackweldwasrepairedandthefuelsupportswerereplacedandareinspectionofthoseitemswasmade.Aftercompletionofthereinspection,.theshroudheadwasinstalledandten-sioned,thevesselheadwassetandthevesselwasfilledtotwofeetbelowtheflangeinpreparationofcontinuingReactorRecirculationFlowtesting.TheRecirculationFlowtestingwascompletedwithnofurtherproblems.ReactorRecirculationpumpswereoperatedtoprovidegustunder50%core(flowtoallowheat-upto135Finordertoachievethermaltighteningoftheshroudheadbolts.Thismethodprovidedaheat-uprateof1F/hr.Vessellevelcontrolwasaccomplishedusingfeedwaterinjectionandreactorwater-cleanupsystemsandmainsteamlinedrainstorejectwater.TemporaryhosebypassedtheRWCU.regenerativeheatexchangerstominimizetheheatloss.ReactorvessellevelwasmonitoredusingthewiderangereactorlevelindicatorLI-1R605usingatemporaryreferencelegconnectedtotheheadventmanifold.Reactortemper-aturewasmonitoredusingthereactorrecirculationloopsuctiontemperaturesrecorder.Thevesselwaspressurizedusinginstrumentairto100psi'andthenwithN2to150psithroughthetemporaryheadventmanifoldsupplyline.Vesselpressurewasmaintainedabove150psithroughouttheflowtest.When135Fwasreached,recirculationpumpspeedswereincreasedto50%,65%,andmaximumspeed(78%toaccomodate100%coreflow).AsetofdatawastakenateachoftheabovespeedsandrecordedinAppendixE,F,6Goftheattachedtestprocedure.Thedualpump100%coreflow35hourrunwasstartedwithcoreflowmain-tainedatthevalueslistedinAppendixHoftheattachedtestprocedure.ThecoolingflowestablishedbythefeedandbleedoffeedwaterandRWCU/MainSteamsystemswassufficienttominimizetheheatupratetokeepvesseltemperaturebelow230Fforthedurationofthe35hourrun(37hoursactual).Bothrecirc-ulationpumpswereoperatedindividuallyforthe14hourruns(16hoursactual)atthesamepumpdriveflowestablishedduringthedualpumprun.Page4of6 VI.Post-InsectionFollowingcompletionoftheflowtesting,thereactorvesselwascooleddownanddepressurized.TheRPVheadwasremovedandthevesselwasdrainedandthepost-inspectionwasconductedbyI&SEandaninspectionteamconsistingoftwoengineers,oneQCInspector,andaphotographerwiththefollowingresults:1.25%oftheincoreguidetubestabilizerbracketshadrustypatchesonthemeventhoughtheyweremadeofstainlesssteel.2.A5"longweldingrodwasfoundontheshroudheadsupport.belowjetpumps//13and814.3.Thelowerriserbraceweldontheshroudsideofjetpumpf/5"and86risershowedevidenceoflackoffusion.4.Therewereseveralmarksontheshroudledgeneartheseismicpins.Theyappearedtobeinstallationmarks.5.Steamseparatorstandpipe(Serial8A460)toshroudheadweldhas"Sugaredburnthrough".6.Anothercrackedtackweldwasfoundonacorespraynozzle.Twoothershadgranulartackweldsandonenozzleonlyhadonetackweld.7.Thecorespraypipingabovejetpump820hadasurfacedefect(inden-tation)<"X3/4"X1/16"deep.8.About50%ofthetopguideboltkeepersandwedgeswereloose(i.e.,theycouldbemovedwithyourhands).9.Severalpeicesofgasketmaterialandducttapewerefoundinvariouscorelocations.SeethePost-InspectionAppendixJ's.10.Theperipheralsinglefuelsupportcastingatlocation'53-12hada4"squareby1/16"deepsurfacedefectinit.ll.Themissing"0"ringretainingclipwasfoundonthejetpumpannulusbetweenjetpumpf/'s15and16.ThesediscrepanciesweredocumentedinPP&LNon-ConformanceReports(NCRs).ThefollowinglistsummarizestheseNCRs:1.PP&LNCRf/80-251:RPVheadandmissingclips2.PP&LNCR880-260:Missingcomponentsoftemporarycover3.PP&LNCR$180'-'261;4.PP&LNCR//80-263:5.PP&LNCR880-320:Missinggasketmaterial"B"recircloopsurfacediscontinuities1.Gougein"B"loopbetweenvalve&elbow2.Gouges(2)in"B"loopbase3.ArcstrikeonbottomofshroudbyjetpumpIllPage5of6
