with your application dated June 28, 1990, as supplemented on July 2,1991; i March 9, 1992; April 15, 1994; and May 30, October 9, and December 7, 1995.

In connection with the license termination, we are enclosing two copies of Amendment No. 12 to Indemnity Agreement No. E-50. Please sign one copy and return it to this office. ,
