v NOTICE OF VIOLATION Commonwealth Edison Company Docket No. 50-455 As a result of the inspection conducted on January 5 - 23, 1987, and in accordance with the " General Policy and Procedures for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1986), the following violation was identified:

Technical Specification 3.0.4 requires that entry into an Operational Mode shall not be made unless the conditions for the Limiting Condition for Operation are met without reliance on provisions contained in the Action Requirements.

Technical Specification 3.5.2 requires that two independent Emergency Core Cooling Systems (ECCS) subsystems shall be operable in Modes 1, 2, and 3.

Each ECCS subsystem shall be comprised of, in part, an operable Safety Injection (SI) pump.

Contrary to the above, on December 16, 1986, Mode 3 was entered with valve 2SI89218 shut rendering the 28 SI pump inoperable, thus making one ECCS subsystem inoperable.

This is a Severity Level IV violation (Supplement I). (455/87004-01(DRP))

The inspection showed that action had been taken to correct the identified violation and to prevent recurrence. Consequently, no reply to the violation is required and we have no further questions regarding this matter.
