I NOTICE OF VIOLATION

. Commonwealth Edison Company Docket Nos. 50-454; 50-455 Byron Station Units 1 and 2 License Nos. NPF-37; NPF-66 As a result of the inspection conducted from February 17 through March 31, 1989, and in accordance with the " General Policy and Procedures for NRC  ;

Enforcement Actions," 10 CFR Part 2, Appendix C (1988), the following violation was identified:

10 CFR Part 50, Appendix B, Criterion XVI requires that measures be established to assure that conditions adverse to quality, such as deficiencies and deviations, are promptly identified and corrected and that these measures assure that the cause of the condition is determined and corrective action taken to preclude repetition.  ;

Contrary to the above, corrective action taken following two separate events, in April 1987 and October 1988 involving deficiencies in control of maintenance work on Limitorque valves, did not preclude a third event from occurring. On January 17, 1989, seal injection water was discovered streaming onto the 2D Reactor Coolant Pump through valve 2CV8355D which had been lifted slightly off of its seat when a licensee contractor was removing the Limitorque operator from the valve. The root cause of this event was that proper isolation points had not been added to the out-of-service for this work activity. Similar unanticipated results occurred during the two previous events and corrective actions taken in response to these events were apparently not effective in correcting the deficiencies and precluding repetition. In addition, investigation following the most recent event revealed that several Limitorque valves scheduled to be worked on during the Unit 2 refueling outage did not have proper isolation points identified in their respective out-of-services, demonstrating multiple examples of a lack of adequate control of maintenance activities involving Limitorque valves.

This is a Severity Level IV violation (Supplement I). (454/89004-02(DRP);

The inspection showed that action had been taken to correct the identified violation and to prevent recurrence. Consequently, no reply to the violation is required and we have no further questions regarding this matter.
