NOTIC 0F VIOLATION

Commonwealth Edison Company Docket No. 50-455 License No. NPF-66 As a result of the inspection conducted on March 13-17, and 22,1989 and in accordance with the " General Policy and Procedure for NRC Enforcement Actions,"
10 CFR Part 2, Appendix C (1988), the following violation was identified:
Criterion V of 10 CFR Part 50, Appendix B, as implemented by Commonwealth Edison Company's Quality Assurance Manual, Quality Requirement 5.0, requires that activities affecting quality shall he prescribed by documented instructions, prccedures, or drawings, of a type appropriate to the circumstances and shall include appropriate quantitative or qualitative acceptance criteria for deter-mining that important activities have been satisfactorily accomplished.
Contrary to the above, Byron surveillance Procedure No. 2BVS 0.5-2. SI2-3, ~~
" Accumulator Back-Up Check Valve Stroke Test," Revision 3, dated January 7, 1989, and performed on March 2,1989, did not contain adequate acceptance criteria to ensure that the accumulator outlet check valves 2SI8948A-D and 2SI8956A-D were full-stroke exercised in accordance with ASME Code Section XI requirements.

This is a Severity Level V violation (Supplement I).
The inspection showed that actions had been taken to correct the identified violation and to prevent recurrence.
Consequently, no reply to the. violation is required and we have no further questions regarding this matter.
