q e ENCLOSURE 1 NOTICE OF VIOLATION Tennessee Valley Authority Docket Nos. 50-259, 50-260, and 50-296 Browns Ferry 1, 2, and 3 License Nos. DPR-33, DPR-52 and DPR-68 During the Nuclear Regulatory Commission (NRC) inspection conducted on October 1-31, 1988, one violation of NRC requirements was identified. The violation involved two examples of i failure to follow procedures in 4.agging out a diesel generator starting air compressor and performing a surveillance instruction on the Radiation Monitoring System.

In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1985), the violations are listed below:

Technical Specification (TS) 6.8.1 requires that procedures recommended in Appendix "A" of Regulatory Guide 1.33, Revision 2, be established, imple-

.mented and maintained. This includes maintenance and surveillance proce-dures.

The TVA NQAM, Part I, Section 2.5, " Instructions, Procedures and Drawings" establishes programmatic requirements to assure that activities affecting quality art prescribed by and accomplished in accordance with documented instructions, procedures, and drawings.

During this inspection the following instances of failure to follow procedures were identified:

1) Site Directors Standard Practice (SDSP) 14.9, Equipment Clearance Procedure, requires component positioning and tagging to be as provided on the clearance sheet. SDSP 14.9 and SDSP 3.15, Indepen-dent Verification, ' require independent verification of the positions of camponents in systems identified in Attachment A of SDSP 3.15, Systems and Components Requiring Independent Verification. The Diesel Air Start System is listed in Attachment A of SDSP 3.15.

Contrary to the above, on October 16, 1988, the licensee failed to properly perform, and to independently verify, the tagging of equip-ment specified on a clearance sheet, in that "A" air start compressor for the Units 1 and 2 diesel generator "C" was tagged instead of the compressor for the "B" diesel generator as specified on the clearance. This was identified through the receipt of an annunciator alarm and resulted in all eight diesel generators being technically inoperable.

that instructions shall be.provided and followed step-by-step for all tasks where operations must be performed in a.specified sequence and reliance on memory cannot be trusted.

Contrary to : the above, on.- October 17, 1988, Steps 7.6.111.1' thru 7.6.111.4" of 2-SI-4.2.~A-10, Reactor Building' and Refueling . Floor .

3 Ventilation Radiation Monitor calibration and Functional.; test, were 1 not performed prior to performing Step 7.6.111.6. This'resulted.in- 1 an inadvertent' Engineered Safeguards Features. actuation.

This is a Severity- Level-IV Violation (Supplement I), and-is applicableLto Units l'and 2.

Where good cause-is shown, consideration will be given to extending the response time.
