i During an NRC inspection conducted on August 3 through September 8, 1988, one violation of NRC requirements was identified. In l accordance with the "General Statement of Policy and Procedure for  !

NRC Enforcement Actions," 10 CFR Part 2, AFpendix C (1988), the  ;

violation is listed colows Criterion y cf Appondix B to 10 CFR Part 50, as implemented by Section 5.0, Revision 0, of the TU Electric Quality Assurance Manual dated February 1, 1988, requires that activities affecting quality shall be prescribed by and accomplished in accordance with i documented instructions, procedures, or drawings.

Paragraph 7.1 of Field Verification Method (FVM) CPE-SWEC-TVM- t EE/ME/IC/CS-090, Revision 5, states: "Those commodities and i l associated attributes identified on Attachments 1-4 of this FVM I shall be reinspected by Quality control in accordance with the ,

referenced specification criteria identified on Attachments 1-4 of l the FVM." Attachment 3 to this FVM shows that location, i i orientation, and welding are to be inspected for structural steel, i f

Contrary to the above, Post Construction Hardware Validation j Program (PCRVP) package CS-090-SG1-773-54-S1, for a safety-related l watertight door, contains Inspection Report 1-0184414 with  !

attributes for location, orientation, and welding incorrectly l marked "N/A" (not applicable). Construction traveler l CE-82-201-8903, the original construction document, specifies, in part that (1) the door assembly identification and location be i verified and (2) welding be used between the door frame and the  ;

associated embeds. The welds provide structural attachment of the door frame to the building walls and/or provide a watertight seal.

The attributes for location, orientation, and welding are i applicable to this PCHVP package.

This is a severity Level IV violation (Supplement II) [

Nuclear Rcquiatory Commission, ATTN: Document Control Desk, Washington, DC. ?o;55, with r copy to the Assistant- Director for Inspection Programs, Comancho Peak Project Division, Office of Special Projects, within 30 days of the date of the letter transmitting this Notico. This reply should be clearly marked as a "Reply to a Nctice of Violation" and should includo for cach violations (1) the reason for the violation if admitted, (2) the corrective steps that have been taken and the results achieved, (3) the corrective steps that will be taken to avoid further violations, and (4) the date when full compliance will be achieved.

If an adequate reply is not received within the time specified in this Notico, an order may bo issued to show cause why the licenso should not be modified, suspended, or revoked or why such other action as may be proper should not be taken. Where good cause is shown, consideration will be given to cxtending the response time.
