APPENDIX A NOTICE OF VIOLATION TV Electric Docket: 50-445/90-34 Comanche Peak Steam Electric Station, Operating License: NPF-87 Unit 1 During an NRC inspection conducteo during July 31 through August 7, 1990, one viciation of NRC requirements was identified. This violation involved an inadequate hot standby operating procedure. In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1993) (Enforcement Policy), the violation is listed below:

Technical Specification 6.8.1.a requires, in p.trt, that written procedures shall be established, implemented, and^ maintained for activities referenced in Appendix A of Regulatory Guide 1.33, Revision 2, February 1978.

Paragraph 2.d of Appendix A of Regulatory Guide 1.33, Revision 2, February. l" 1978, requires that a general plant operating procedure for operations in hot standby be established, implemented, and maintained. This is acconip11shed at i CPSES Unit 1 by Integrated Plant Operating Procedure IPO-007A, Revision 5,

" Maintaining Hot Standby " i Contrary to the above, IPO-007A was inappropriate to the circumstances on July 30, 1990, when Unit I was operating in bot standby in _that IPC-Gi.JA did l not require the periodic draining of 14olated MSIV upstream drippots wit.h the l MSIVs shut. As a result, water fermed f rom condensed main steam, backei up to the upstream side of the No. 3 & Joheric relief valve (ARV) and, appurantly, 3 caused erratic ARV performance upe 'ening and subsequent damage to insulation a'o lighting fixtures in the ARV room. 1 9
