APPENDIX A NOTICE OF VIOLATION TU Electric Docket: 50-445/89-67 Comanche Peak Steam Electric Station Permits: CPPR-126 Unit 1 During an NRC inspection conducted August 2 through September 5, 1989, a violation of NRC requirements was identified. In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1989), the violation is listed below:

10 CFR 50, Appendix B, Criterion XVI states, in part, that measures shall be established to assure that conditions adverse to quality, such as deficiencies, are corrected.

By letter (TKX-89272) dated May 25, 1989, from William J. Cahill, Jr.

to the NRC, the corrective action for reportable deficiency (pursuant to 10 CFR 50.55[e]) CP-88-33, " Emergency Diesel Generator (EDG)

Voltage. Output" was described. The corrective action included revising the system operating procedure to require an operator to take a certain, specified action during EDG shutdown to ensure EDG availability.

Centrary to the above, the measures established did not assure that deficiency CP-88-33 was corrected in that an operator stationed at the "B" EDG could not take the action specified by the revision to the procedure because of an inadequate procedure (the procedure used the wrong nomenclature for an indicating light) and inadequate training (the operator was not trained as to which indicating light to use).

This is a Severity Level IV violation (Supplement II)

No reply to this Notice of Violation is required because the inspection showed that corrective action had been taken to correct l

the violation and to prevent recurrence. We have no further questions regarding this matter.
