t APPENDIX A NOTICE OF VIOLATION TU: Electric Dockets:
50-445/89 ' 445/89 1 ComancherPeak; Steam' Electric Station Permits:
CPPR-126 Unit 1, Glen Rose,-Texas CPPR-127-During an NRC in'spection conducted on May 3 through June!6, 1989, a
. violation.of'NRC requirements was identified.
In accordance with the " General Statement of Policy and Procedure fer.NRC Enforcement Actions," 10 CFR Part'2, Appendix C (1988), the violation is listed below:
Criterion V of Appendix B to 10 CFR Part 50 states, in part, that activities affecting quality shall be' prescribed by documented procedures ofia. type appropriate to the circumstances, and'shall be accomplished in-accordanceLwith these procedures.
Paragraph 6.2.7;9.1of Procedure TDA-303, Revision 0, " Conduct of Testing,". states, " Testing shall be performed in accordance with-L
' approved test procedures and instructions."--Paragraph 6.1.6 of TDA-306,. Revision 0, " Control'of Deficiencies and Nonconformances,"
' states in part, "When an approved test. procedure / instruction is not complied with, a TDR shall be processed.
Contrary to these requirements on May 10, 1989, during the conduct of.the Remote Shutdown Capability and Loss of Control Room Habitability. tests, the Shift Test Engineer failed to issue a' test q
deficiency report to document a valve misalignment that occurred during the testing.
This failure to document a deficiency is a Severity Level IV violation (Supplement 1) (445/8937-V-04).

This reply should be clearly marked as a
" Reply to a Notice of Violation" and should include for each violation:

violations, andz.(4) the date when full compliance'will be achieved.
If.an' adequate reply is not received within the time specified in

this Notice, an order!-may be issued to show cause why the license should not.be modified, suspended, or. revoked or,why such other action as may'be proper.should not be taken.
Where good cause is shown, consideration will be!given to extending the~ response time.
