APPENDIX A NOTICE OF VIOLATION 10 Electric Docket No. 50-445 Comanche Peak Steam Electric Station License No. NPF-87 Construction Permit No. CPPR-127 During an NRC inspection conducted on November 9-13, 1992, a violation of NRC requirements was identified.

In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C, the violation is listed below:

10 CFR 20.201(b) requires that each licensee make or cause to be made such surveys as (1) may be necessary for the licensee to comply with the regulations in this part and (2) are reasonable under the circumstances to cualuate the extent of radiation hazards that may be present.

Contrary to the above, on November 7, 19'J2, workers performing in service inspection preparation work in the Unit I reactor containment building removed insulation from piping and used a wirebrush to removc corrosion from a valve without first having the radiation hazards associated with this work evaluated through the analysis of contamination surveys.

Ihis is a Severity Level IV violation (Supplement IV).(445/9256-01)

Pursuant to the provisions of 10 CFR 2.201, TV Electric is hereby required to submit a written statement or explanation to the U.S. Nuclear Regulatory Commission, ATTH: Dot.ument Control Desk, Washington, D.C. 20555 with a copy to the Regional Administrator, Region IV, and a copy to the NRC Resident inspector at the facility that is the subject of this Notice, within 30 days of the date of the letter transmitting this Notice of Violation (Notice).

This reply should be clearly marked as a " Reply to a Notice of Violation" and should include for each violation:

(1) the reason for the violation, or, if contested, the basis for disputing the violation, (2) the corrective steps thathavebeentakenandtheresultsachieved,(3)thecorrectivestepsthat will be taken to avoid further violations, and (4) the date when full compliance will be echieved.

If an adequate reply is not received within the time specified in this Notice, an order or demand for information may be issued as to why the 1: ;ense should not be modified, suspended, or revoked, or why such other action as may be proper should not be taken. Where good cause is shown, conhideration will be given to extending the response time.
