NPDES Non-Compliances for Quad-Cities Generating Station IL-0005037

Dear Sirs:

l In accordance with Standard Condition 12 we wish to submit l the following notice of non-compliances:

DISCIIARGE: 006 and 007 Open Cycle Diffusers PARAMETER AND PERMIT LIMIT: 'Jotal Residual Chlorine / Total Residu.1 Halogen Daily Max 0.2 mg/L NON-COMPLIANCE AND DATE: On 25JUL88, the measured total '

residual oxident exceeded the 0.2 mg/L limit. The maximum residual oxident measured at the river was 0.62 mg/L.

CAUSE: The cause of the problem has been attributed to a mechanical failure of the oxident f6ed pump. An trnal check valve in the pump failed 'na 11 owed the oxident to be injected at about three times the normal dosage, n of sa@ wf $

t e- .s CORRECTIVE ACTION: The sodium hypochlorite tank was isolated' as soon as the increased flow was discovered. The maximum time at the increased flow was 3 1/2 hours or a loss of zul additional 1350 gallons of sodium hypochlorite.

The oxident' feed pump will.be repaired to correct the check valve problem.'.

After the pump has been repaired, the pump feed rate will be measured to ensure the correct oxident flow rate. ,

The system is shut down and will-not be operated until the check valve is repaired and:testad.
