NPDES Non-Compliances for Quad-Citiec Generating Station IL-0005037 Dear Sirst In accordance with Standard Condition 12 we wish to submit the following notice of non-compliances:

DISCIIARGE: 006 & 007 Open Cycle Diffusers PARAMETER AND PERMIT LIMIT: Total Residual Chlorine / Total Residual Halogen Lead Limits Daily Max 400.5 lbs/ day.

NON-COMPLIANCE AND DATE: Total Residual Chlorine / Total Residual Halogen was 485 lbs/ day on 7/25/88.

CAUSE: The cause of the problem has been attributed to a mechanical failure of the oxident feed pump.

An internal check valve in the pump failed and allowed the oxident to be injected at ebout three times the normal dosage.

CORRECTIVE ACTION: The sodium hypochlorite tank was isolated as soon as the increased flow was discovered. The maximum time at the increased flow was 3 1/2 hours or a loss of an additional 1350 gallons of sodium hypochlorite. g p

The oxident feed pump will be repaired to correct the check valve problem. After the pump has been repaired, the pump feed rate will be measured to ensure the correct oxident flow rate.

The system is shut down and-will not be operated until the check valve is repaired and tested.
