f ENCLOSURE 1 NOTICE OF VIOLATION Tennessee Valley Authority Docket Nos. 50-259, 50-260, and 50-296 Browns Ferry 1, 2 and 3 License Nos. DPR-33, DPR-52, and DPR-68 During the Nuclear Regulatory Commission (NRC) inspection conducted on June 12-30, WSS a violation of NRC requirements was identified. The violation ir.volved 'ailure to initiate a Condition Adverse to Quality Report (CAQR) followinr, an inadvertent overload of the 1/2 "D" Diesel Generator. In a:cordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1935), the violation is listed below:

10 CFR 50, Appendix P. Criterion V requires that activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings.

The Nuclear Quality Assurance Manual, Part I, Section 2.16, Step 2.1.1.F implements this requirement by specifying that a condition adverse to quality shall be documented on a CAQR for items which have been cubjected to conditions for which they have not been designed such as overpressure, overvoltage, overheating, overstressing, or excessive environmental conditions. Further, Step 2.1.1.A requires a CAQR be initiated for items

that have failed or malfunctioned.

Contrary to the above, a CAQR was not generated to document a condition adverse to quality that occurred during the conduct of Special Test 88-09, Diesel Generator Governor and Voltage Regulator Cslibration, on June 21,

1988. Diesel Generator "0" was inadvertently overloaded for a period of 30
l seconds during which time the control room indications of kilowatts (KW),

4 kilovars (KVAR), and imperage ( AMPS) were of tscale high. The malfunction

was, in part, a result of a failed GE SEM switch in the diesel generator breaker comparteent.

This is a Severity Level IV violation (Supplement I) and is applicable to all three units.

Pursuant to the provisions of 10 CFR 2.201, Tennessee Valley Authority is hereby required to submit a written statement or explanation to the Nuclear Regulatory Commission, ATTN: Docu ent Control Desk, Washington, DC 20555.

with a copy to the Director, Office of Special Frojects, and a copy to the NRC Resident Inspector, Browns Ferry, within 30 days of the date of the letter transmitting this Notice. This reply shou'd be clearly marked as a "Reply to a Notice of Violation" and shoulo include [for each violation): (1) admission or denial of the violation (2) the reason for the violation if admitted, (3) the corrective steps which have been taken and the results achitved. Where good cause i s shown, consideration will be given to extending the response time. If an adequate reply is not received within the time speci'ied in this Notice, an order may be issued to show cause why the license should not be modified, suspended, or revoked or ::hy such other action as may be proper should not be taken.
