NIS-1 REPORT ABSTRACT

10. This submittal is for ISI examinations performed during Oyster Creek's refueling outage 16R., These examinations completes the second period requirements for the " Third Inspection Interval." See the attached, " Ten Year Inspection Scope Statistical Report" for details.

The ISI scope initially scheduled nine category B-D examinations. Only seven were completed. Two Core Spray Nonle to Vessel Welds was determined inaccessible due to the bio-shield opening configuration. These welds have been re-scheduled for refueling outage 18R. During outsige 18R the Feed Water Nonles are scheduled for ISI examinations. At that time the bio-shield opening for the Feed Water Nonles would be open also providing a greater access to the Core Spray Nozzles Seven category B-F examinations were scheduled, and only five were completed. Two Isolation Condenser Nozzle to Safe-end welds were not performed because of material ,

concerns with the calibration block. These welds were re-scheduled for examination durmg i refueling outage 17R.  !

Sixteen IGSCC examinations were applied to ISI Section XI credit. Five for category B-F and eleven for category B-J. These sixteen examinations were perfonned utilizing PDI ,

qualified procedures and technique. Although the examinations were performed by an )

approved PDI technique, the examinations did not fully comply with Section XI Appendix  !

111 from the 1986 or 1992 Edition. Relief Request Ri1 Rev. I was applied for crediting j these IGSCC exams toward Section XI. These exams are noted on the NIS-1 Report in the remarks colunm. l

11. Reactor Vessel: Six vessel internals components were reported to have indications. The Steam SEPARATOR, Lower Shroud Conical Support, Steam Dryer Lug, Shroud Vertical Weld, Top Guide, and the Feedwater Sparger Adjusting Nut / Bolt.

Indication (1) was NOZZLE TO SAFE END rounded,7/16 inch. ans indication (2) was hnear,5/8/ inch. The indications were determined to be caused by presious mechanical work on the OD. Both indications were removed by light filing. A re-examination was performed and accepted.

3113 B-J B9.12 t'-Z-3-0034B WELD 212 SUR An indication 1.75 inches in length was recorded by dye penetrant. The INTRADOS OF ELBOW indication had the appearance of having been blended. The indication was in the bade material of the elbow. In accordance with Section XI paragraph IWB-3514.3(b), a ut examination was performed and determined the indication acceptable per Table IWB-3514-2. This weld was re-scheduled for examination during the next refueling outage 17R.

658 B-J B9.32 NG-A-000A WELD 223 SUR Ten surface indications were detected by dye penetrant. Seven ELBOW TO BRANCli indications were classified as linear, and three were rounded. The 2*OD acceptance criteria from Section XI Table IWB-3514-2 is 0.25 inches in length for u all thickness between 1.00 inche to less than 2.00 inches.

The seven knear were all within the allowable for base material 1.219 inches. The rounded indications were also determined to be acceptable.

This weld was re-scheduled for examination during the next refueling outage 17R.

1779 C-F-1 C5.11 NZ-3-0002 WELD 212 SUR A shallow indication 1.00 inches knear was detected by dye penetrant in TEE TO PIPE the base metal on the tee. This indication was determined to be nim service induced. In 10 mils increments the indication was removed by grindmg and IFT exams. A UT thickness was performed to venfy remaining walt 4122 C-F-2 C5.90 NZ-2-0276 WELD 212 SUR A shallow indication 1.2 inches linear was detected by magnetic particle PIPE TO ELBOW at the toe of the weld. This indication was removed by hand filing.

I13 F-A F1.10 212-BP-NZ-3-lil(B)-0003 212 VT-3 (1) Lock nut loose against top of sprmg can. (2) i Beam is bent do* n on VARIABLE SPRING upper outer edges both sides of beam. Item (1) loose lock nut was hand FLANGER tighten. Item (2) was determined to be cause by use of chain fall to hfe valve v-2(M)017. Review of the support calculations, C-1302-212-5320-025 Rev. 3 Sheets 27 to 36, determined that the maximum stress in the support is approx. 20% of the allowable stress. By review of the support drawing it was determined that the location of the bent flanges was not the entical stress hication. The stresses are much lower at the interior h> cation. Since the bends occurred at the outer edges only a porti m of the flange was determined to be effective to resist bendmg . The support was determined to be acceptable for service. This support was re-scheduled for exanunation during the next refuelmg 17R.

'2291 F-A Fl.20 241-BP-NQ-2-R4-0071 241 VT-3 Bent threaded rmi. The rod is oriented in the honzontal. Root cause SWAY BAR evaluated was deternuned to be externalinfluence. The rmt was replaced.
