Pursuant to the requirements of 10CFR, Part 21, Reporting of Defects and Non-Compliance, attached is a report made by Magnetrol's Deviation Evaluation Board.

This report details a product defect found by audit at the TVA Brown's Ferry Plant; and, provides a list of users of identical or similar items.

Further, this report identifies Magnetrol's action to remedy the defect; and, notify the nuclear facilities.

The DEB has determined that this incident is reportable to the NRC under 10CFR Part 21.

The attached report describes the incident for you to prepare your report to the NRC. ,

N MAGNETROL INT E R N ATIO N A L DEVIATION EVALUATION BOARD MEETING BASIC COMP 0NENT: Model 402 Series Level Control used for Nuclear Safety Related Application DEFECT: Non-Compliance:

The Seismic Qualification requires that the enclosing tube nut should be torqued to 200/225 foot /lbs.

Model 402 Series controls have been shipped to customers with-out the torque check.

Without the proper torque, the enclosing tube nut could loosen during a seismic event causing a leak; or, loss of pressure of the process fluid.

A list of customers is attached showing Model Number, Quantity and Serial Numbers.

Tennessee Valley Authority (Browns Ferry Plant) is already notified of this deviation; and, will respond with confirmation of corrective action by April 24, 1986.

Other customers will be notified by April 28, 1986; and, will be requested to confirm torque check in writing.

This condition is only a concern during or after earthquake induced vibrations.

There is no cause for concern during normal plant operation.
