Attachment LER NUMBER 50-368/81-045/03L-0 EVENT AND DESCRIPTION AND PROBABLE CONSEQUENCES (continued) reported in LER 79-040. Occurrences relating to control valves similar to 2CV-1036 were reported ir LER's79-035, 79-043,79-051, 79-068,79-088, 79-089,79-090, 79-092, and 80-003.

CAUSE DECRIPTION AND CORRECTIVE ACTIONS (continued) operation resulting each time. The valve was declared operable and returned to service. An evaluation pertaining to failures of this type valve had been initiated earlier and is still in progress.
