Inoperability of Accident >

Honitor Exceeds 72 Hours  :

AECH-89/0194  :

i The Turbine Building ventilation exhaust accident range radiation monitor was removed froin service on October 10, 1989 to perform a surveillance as required  ;

by Technical Specification Table 4.3.7.5-1. The monitor was declared inoperable l after sample flow calculations could not be completed for Channel 4 of the ,

monitor. l An investigation into the problem revealed the purge inlet valve and a grab sample valve were leaking by. Inleakage from these normally closed valves, i caused the calculated flow to fall outside of specified ranges. Due to procurement problems, valve replacement was delayed. The purge inlet valve ,

was replaced and the grab sample valve was capped. The monitor retested '

satisfactory and was restored to service on October 20, 1989. ,

This Special Report is being submitted pursuant to Action 61 of Technical Specification Table 3.3.7.5-1 since the instrument channel inoperability exceeded 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br />. The preplanned alternate method of monitoring as required by Action 81 was to utilize procedure 06-CH-ID17-V-0058 if the General Electric (GE) Noble Gas Effluent Monitor exceeded a specified count rate. The GE -

Noble Gas Effluent Monitor was operable during this time period and did not exceed the specified count rate.  :
