Additional Comments

1. There were no subversion attempts during the 2013 reporting period.
2. For the 2013 reporting period, all blind specimens submitted to PBNP's HHS-certified laboratory yielded expected results.
3. *On November 22, 2013, one (1) for-cause observed behavior (alcohol only) test was conducted on a licensee employee due to the smell of alcohol. The breath alcohol test was negative. No drug test was required, and the individual was allowed to go back to work in accordance with 10CFR26.77(b)(1 ).

Reportable Events There were no reportable events for Point Beach Nuclear Plant as defined in 10 CFR Part 26.719 during this reporting period.

2 There were five fatigue assessments completed during 2013.

3 For Cause (Non-Outage)- An Operator was sent home pending drug and alcohol test results 4 For-Cause (Outage) An Engineering Contractor was found sleeping during his break. The individual 5 remained at work but was coached and provided strategies for obtaining adequate rest.

6 Post-Event (Non-Outage) A Security Officer went off the road and struck a sign when trying to avoid a deer in 7 the road . The fatigue assessment determined the individual was alert.

8 Post-Event (Non-Outage) A maintenance individual received an injury moving heavy equipment. The fatigue 9 assessment determined the individual was alert.

10 Post-Event (Non-Outage) A Security officer was involved in a vehicle accident driving through the owner 11-20 controlled area. The fatigue assessment detenmined the individual was alert.
