Attachment tcLERl04/01T-0 Rochester GasandElectricCorporation R.E.GinnaNuclearPowerPlant,UnitNo.1DocketNo.50-244OnMay6,1980duringperformance ofpostinstallation hydrostatic pressuretestingofthereactorvesselremotelyoperatedheadventsystemitwasfoundthattheDCswitcheswhichsupplypowertobothtrainsofthelowtemperature overpressurization protection controlofthepressurizer poweroperatedreliefvalveswerefoundintheOFFposition, withnoReactorCoolantSystemventopenasrequiredbyTechnical Specification 3.15whenthesystemtemperature islessthan330F.Thiswasdiscovered whentheOperations Supervisor, duringatourofcontainment, investigated thesoundofventingfromthenitrogensystemforthepoweroperatedreliefvalves.Areviewofpreviousactivities showedthatat0115hoursthatmorn-ingprocedure S-29.2"Charging theReactorVesselOverpressure Protection SystemAccumulators withN2"wasperformed.

TheDCsupplyswitchesmustbeclosedtoallowthenitrogentochargetheseaccumulators.

At1115hoursamemberoftheResultsandTestGroup,inpreparing forthehydrostatic pressuretestofthenewlyinstalled headventsystem,notedthattheseswitcheswereopen,butwasnotawarethatthemodification beingtestedhadbeenaddedtothesamesourceofpowersupplying theoverpressure pro-tectionsystem.At1400hoursamemberoftheResultsandTestGroupinsertedfusesandclosedtheswitchesinordertoperformthehydrostatic pressuretestoftheheadventsystem,andaftercompletion reopenedtheswitchesandremovedthefuses.Thisresultedinventingoftwosmallnitrogenaccumulators intheoverpressure protection system,heardbytheOperations Supervisor.

AfterhenotifiedtheControlRoomofthecondition, thesystemwasrestoredtooperability, andatthattimeitbecameknownthatboththeoverpressure protection systemandtheheadventsolenoidvalvesarepoweredbythesameDCswitches.

Thelowtemperature overpressure protection systemwasprovidedtoinsurethattemperature-pressure requirements inconsideration ofreactorvesselfracturetoughness arenotexceeded.

Infurtherrecognition oftheserequirements thecontrolswitchesforthesafetyinjection pumpswereinthePULL-STOP positionandthereactorcoolant-steam generator temperature differences werevirtually zerosothatpotential causesofoverpressure transients wereminimized andnohazardous consequences resulted.

TheDCpowerswitcheswouldhavebeenreturnedtothecorrectpositionuponcompletion'of workontheheadventsystem.Thepersonnel onthenightshiftanddayshiftwerequestioned aboutwhowasgiventhekeyfortheDCpanel.Theonlypersonswhotheyremem-beredgivingthekeytowerethetwotestindividuals mentioned above.How-ever,sincethekeyiscommontoseveralelectricpanels,itispossiblethatotherkeysareavailable.

Theoperators donotrecallseeingotherpersonnel intheDCpanels.

Attachment tcLER804/01T-0 Thestationholdrecordswerechecked.TheswitcheswererestoredtoserviceonMay2,1980,withnootherholdsperformed onthissystemsincethatdate.Procedure 0-7Alignment andOperation oftheReactorVesselOver-pressureProtection Systemwasmodifiedtorequireverification thattheDCpowerswitchesareclosedpriortoputtingthesystemintoservice.ThelocksfortheDCpanelswillbechangedinordertolimitthedistribution ofkeystopreventrecurrence.
