OnOctober24,1997,theplantwasinMode6withthereactorcoolantsystem(RCS)beingmaintained atatemperature ofapproximately 110degreesFanddepressurized andvented.Theplantwasshutdownforrefueling.

Acontainment (CNMT)purgewasoccurring, andotheractivities wereinprogressinpreparation foropeningtheprimarysidesofthesteamgenerators (SG).Atapproximately 1900EST,allunnecessary personnel hadbeenremovedfromtheCNMT,inanticipation ofremovingtheSGprimarymanwaydiaphragms.

Whenthediaphragms wereremoved,radionoble gasconcentration increased inCNMT.DESCRIPTION OFEVENT:A.DATESANDAPPROXIMATE TIMESOFMAJOROCCURRENCES:

October24,1997,1919EST:Eventdateandtime.October24,1997,1919EST:Discovery dateandtime.October24,1997,1920EST:ControlRoomoperators verifyallcontainment ventilation isolation functions haveoccurred.

October24,1997,2125EST:Containment ventilation isolation isresetandcontainment purgeresumed.B.EVENT:OnOctober24,1997,radionoble gasconcentration wasincreasing inCNMT,duetoremovingtheSGprimarymanwaydiaphragms.

Atapproximately 1919EST,CNMTgasradiation monitorR-12(partoftheCNMTradiation monitoring system,orRMS)reacheditshighalarmsetpoint.

WhenR-12alarms,itresultsinaCNMTventilation isolation (CVI)signal.WhentheCVIoccurred, thefollowing MainControlBoardannunciators werereceived:

A-25(Containment Ventilation Isolation) andE-16(RMSProcessMonitorHighActivity).

TheControlRoomoperators, responding totheaboveannunciators, observedthatR-12wasonalarm.TheControlRoomoperators immediately referredtoAlarmResponseprocedures AR-A-25andAR-RMS,andverifiedthatallCNMTventilation isolation valves(thatwereopen)closedasdesigned, andthatCNMTpurgeandexhaustfanstripped.Theyperformed theapplicable actionsofthealarmresponseprocedures.

Afterthissetpointwastemporarily increased, theCVIsignalwasreset,andtheControlRoomoperators resumedtheCNMTpurgethathadbeeninprogress.

Thiseventwasimmediately apparentduetoMainControlBoardannunciator alarmsandCVIpositionindication ontheMainControlBoard.F.OPERATORACTION:TheControlRoomoperators responded totheeventbyperforming theapplicable actionsofalarmresponseprocedures, andotheractionsasdeemednecessary.

TheyverifiedpropervalvepositionforallCVIvalves.TheControlRoomoperators notifiedhighersupervision andtheNRCresidentinspector.

TheShiftSupervisor subsequently notifiedtheNRCper10CFR50.72(b)(2)(ii),non-emergency fourhournotification, atapproximately 2243ESTonOctober24,1997.G.SAFETYSYSTEMRESPONSES:

ThishigherthannormalRCSactivitywasduetofueldefects.IV.ANALYSISOFEVENT:Thiseventisreportable inaccordance with10CFR50.73,LicenseeEventReportSystem,item(a)(2)(iv),whichrequiresareportof,"Anyeventorcondition thatresultedinmanualorautomatic actuation ofanyengineered safetyfeature(ESF)".TheCVIduetoR-12alarmwasanautomatic actuation ofanESFsubsystem.

Anassessment wasperformed considering boththesafetyconsequences and.implications ofthiseventwiththefollowing resultsandconclusions:

Therewerenooperational orsafetyconsequences orimplications attributed totheCVIbecause:TheCVIoccurredinMode6,whentheCVIisnotrequiredtobeoperable.
