Page 1 of 1 Docket No. 50-338 Report No. LER 78-120/1T-0 Description of Event An inspection of the station battery racks on November 10, 1978, indicated that the number and placement of the tie rods in battery rooms 1-2 and 1-4, were incorrect as per the manufacturers drawings. The NRC Region II office was notified within 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> of the tie rod deficiencies in battery rooms 1-2 and 1-4. A drawing which did not apply to battery room 1-4 was used during the November 10, 1978 inspection. A subsequent inspection of the station battery racks on November 13, 1978 revealed that only the tie rods ,

in battery room 1-2 were incorrectly installed.

An inspection of the station battery racks on November 17, 1978 revealed additional construction deficiencies. The racks in battery rooms 1-4, 1-2 and 1-1 are incorrectly mounted to the wall, the 36 cell battery rack ia

(~ battery room 1-3 is missing three rail support bolts and the 24 cell bactery rack in battery room 1-3 is missing a diagonal strut.

The events described above are reportable pursuant to Specification 6.9.1.8.i.

Probable Consequences of Event The probable consequences of the construction deficiencies on the structural integrity of the battery racks during a seismic event are being evaluated.

Because the battery racks were designed with a large margin of safety and constructed as designed with the limited exceptions noted above, failure of the racks during a seismic event is not expected.

The public health and safety are not endangered.

Cause of Occurrence The battery racks were improperly assembled and installed during construction. i

Immediate Corrective Action No immediate action was taken. Tie rods were installed on the battery racks' in battery room 1-2 as per the manufacturers drawings on November 17, 1978.

Scheduled Corrective Action Construction deficiencies will be corrected. Correcting the deficiencies might require removing battery banks from service. An appropriate time for implementing corrective actions is being evaluated.

Actions Taken to Prevent Recurrence Actions to prevent recurrence are not required.
