Anchorage
: 1. Is the anchorage configuration verification required (i.e., is the item oneof the 50% of SWEL items requiring such verification)?
This is an anchored component, but due to lead shielding, one of theanchor bolts cannot be visually inspected, therefore, this is not part ofthe 50% anchorage verification.
: 2. Is the anchorage free of bent, broken, missing or loose hardware?
Three visible anchors are in good condition.
Y[-7 NZYE Nr7 UD7 N/A[l3. Is the anchorage free of corrosion that is more than mild surfaceoxidation?
Three visible anchors are free of oxidation.
: 4. Is the anchorage free of visible cracks in the concrete near theanchors?A surface crack is visible near one anchor, but since all bolts are cast inplace in the foundation, this is not a structural deficiency.
Although oneanchor bolt and nut is not visible, the concrete next to this and all otheranchors is visible and free of cracking.

Area walk-by is AWC-04.P205 is not directly labeled in the field. Label request was entered in eSOMS to have the item labelcreated. The turbine and the pump are on different skids. However the concrete pedestals are quite rigid andare tied together.
Therefore there is no potential for differential movement between pump and driver. This was confirmed by the SEWs package.Evaluated by: Ga Sweder / A I.-.-0Date: 10-15-12Laura Maelav YWYLAý "fnCa DQUA-10-15-124EN-DC-168 REV 0 Engineering Report No. PNPS-CS-12-00001, Rev. 1Attachment CPage C35 of C413ATTACHMENT 9.6 SEISMIC WALKDOWN CHECKLIST FORMSheet 4 of 5Status: YZ No- U]Seismic Walkdown Checklist (SWC) SWELl- 012Equipment ID No. P205 Equip. Anchorage
: 1. Is the anchorage configuration verification required (i.e., is the item oneof the 50% of SWEL items requiring such verification)?
Due to lack of visual verification of rear cabinet anchors, this was notchosen for the 50% verification.
: 2. Is the anchorage free of bent, broken, missing or loose hardware?
Anchorage at the back of the panel is not visible without breaking theplane of the cabinet, but anchors at the front of the cabinet are in goodcondition.
: 3. Is the anchorage free of corrosion that is more than mild surfaceoxidation?
Anchor bolt on outside of cabinet securing a clip is corroded and thethreads of the anchor are no longer visible.
CR-PNP-2012-04509 waswritten to correct the issue. The corrosion is only on the surface with noapparent loss of metal thickness.
Therefore there is no loss of strengthand the hold down plates will perform their design function.
The oneanchor bolt is fused to the nut by general but localized corrosion.
Although they are fused there is no flaking of metal or any apparentreduction in bolt or nut diameter.
Therefore there is no loss of strengthand the anchor will perform its design function.

Under the valve, two pins were found duct taped to a stanchion above a spring can. This wasdiscussed with operations and engineering management, and was determined to be an acceptable practice for holding the pins in place; therefore, it is not a seismic interaction or a housekeeping concern.SWEL1-018 is on AWC-007.Evaluated by: David Small 0 J/ I 'AHalie ArozDate: 10/09/2012 10/09/2012 EN-DC-168 REV 0 Engineering Report No. PNPS-CS-12-00001, Rev. 1Attachment CPage C62 of C413ATTACHMENT 9.6 SEISMIC WALKDOWN CHECKLIST FORMSheet 4 of 5Status: Y[ Nr I U--ISeismic Walkdown Checklist (SWC) SWELl- 018Equipment ID No. MOIOOI-36A Equip. Detailed inspections were performed and photos taken of 2 of the 145 C V305-127 valves: HCU 26-39(West) and on HCU 42-19 (East). The remainder of the CV305-127 valves were inspected for overallconfiguration compliance along with their HCU assembly as part of the area walk-by.

Note that thisincluded a detailed visual inspection of the anchorages of all HCUs.SWELI-O05 is on AWC-014 andAWC-016.

Anchorage

P205 Equip. Anchorage

1. Is the anchorage configuration verification required (i.e., is the item oneof the 50% of SWEL items requiring such verification)?

Due to lack of visual verification of rear cabinet anchors, this was notchosen for the 50% verification.

2. Is the anchorage free of bent, broken, missing or loose hardware?

Anchorage at the back of the panel is not visible without breaking theplane of the cabinet, but anchors at the front of the cabinet are in goodcondition.

The paint is stripped around the welds between the valve body and the piping. There is mild surfacecorrosion in the area, but this is not considered an adverse seismic condition.
