Dear Mr. MacDonald:

We plan on renewing certificates of compliance numbers 9126, 9127 and 9128 which expire on 31 October 1988.

We request a six month extension on these certificates in order to afford us sufficent time to put together consolidated applications for these three packages.

The consolidated application will include updated information relating to Amersham Corporation as the manufacturer and distributor of these packages.

We will submit the consolidated applications by 30 April 1989 for your review and renewal of these certificates.

I have enclosed an application fee of $150.00 to cover the cost of this 6 month renewal for each certificate.
