Notice is hereby provided that your Quality Assurance (QA)
Program Approval for Radioactive Material Packages, Approvali No. 0648 expired on January 31, 1993.
If you are an NRC licensee and conduct activities under the General Licenses of Subpart C of 10 CFR Part 71, or an Agreement 1 State Licensee subject to the requirements of 10 CFR Part 71, as required by 10 CFR S 150.20, a QA Program approved by'the Commis-sion as satisfying the provisions of Subpart H of 10 CFR Part'71-is required.
Since you no longer have a valid QA Program Approval, the General Licenses of Subpart C of 10 CFR Part.71 and-registration under the provisions of 10 CFR S 71.12, to transporte or deliver licensed material to a carrier.for transport is termi-nated.
This includes your registration as a user of Certificate of Compliance Nos. 6717, 9021, 9032, 9033, 9126, 9128,.9148, 9156, and 9157.
However, we.have registered you for the use of'this package under the provisions of 49 CFR S 173.471.
Please note that records required by 10 CFR S 71.91'are t'o be' retained for a period of three years.
