Please find enclosed two (2) copies of Scientific Inspection Technology, Inc.'s Quality Assurance Program for Transportation and/or Packing of Radioactive Material (RSPM-2). Also we have enclosed two (2) copies of Scientific Inspec-tion Technology, Inc. 's Radiation Safety Procedure Manual (RSPM-1).
RSPM-2 shall be used in conjunction with RSPM-1 to insure Scientific Inspection Technology, Inc.'s Quality Assurance Program for the Transportation and/or Packaging of Radioactive Material as per the scope of the description submitted herein.

CONDITIONS (continued)
- 12. The licensee shall comply with applicable provisions of 1200-2-4,1200-2-5,1200-2-8, and 1200 10 of " State Regulations for Protection Against Radiation".
- 13. The authorized place of use is limited to temporary job sites of the licensee throughout the State of Tennessee. (This condition does not prohibit use in other states under reciprocity privileges which may be granted by the regulatory agency having jurisdiction.)
- 14. Radioactive material authorized by this license shall be stored at Industrial Laboratories, Inc.,
600 East 10th Street, Chattanooga, Tennessee.
- 15. The licensee shall exercise appropriate administrative control to ensure that no person will use or personally supervise the use of radioactive material until such person has received instruction in applicable Sections of 1200-2-4,1200-2-5,1200-2-8, and 1200-2-10 of " State Regulations for Protection Against Radiation", the licensee's written instructions referred to in Condition 18, and in the provision of this license as to possession and use within Tennessee.

- 16. The licensee is authorized to receive, possess, and use sealed sources of Iridium-192 or Cobalt-60 where the radioactivity exceeds the maximum amount of radioactivity specified in Item 9 of this license provided:
A.
Such possession does not exceed the quantity per source specified in Item 9 by more than 20% for Iridium-192 or 10% for Cobalt-60; B.
Records of the licensee show that no more than the maximum amount of radioactivity per source specified in item 9 of the license was ordered from the supplier or transferor of the radioactive material; and C.
The levels of radiation for radiographic exposure devices and storage containers do not exceed those specified in 1200-2-8.04(1) of " State Regulations for Protection Against Radiation".
- 17. Written instructions entitled Attachment I (Survey Meters), Attachment 2 (Radiation Safety Procedures Manual), and Attachment 3 (Radiation Safety Training Manual), shall be followed, except where they may conflict with " State Regulations for Protection Against Radiation", or specific conditions of this license. A copy of these instructicns shall be supplied to each individual using or having responsibility for the use of radioactive material. Any changes in these instructions shall have the prior approval of the Tennessee Department of Public Health.
- 18. Pursuant to 1200-2-8.04(5) of " State Regulations for Protection Against Radiation", the licensee is authorized to perform tests for leakage and/or contamination of the sealed sources authorized by this license in accordance with procedures contained in Attachment 2 (Radiation Safety Procedures Manual).
- 19. The licensee shall maintain complete and accurate records of the receipt and disposal of radioactive material. The licensee shall, for radioactive material no longer useful for any purpose and for any equipment or supplies contaminated with such material for which further use and decontamination is not planned, define those materials as radioactive waste and treat them as such in accordance with the following provisions:
A.
Radioactive waste material shall not be stored with non-radioactive waste.
B.
A written record of all radioactive waste material shall be maintained until it has been determined by a suitable survey or radioassay that it has decayed to background levels or until it has been shipped to an authorized recipient in accordance with all applicable regulations. Accountability of radioactive waste material prepared for shipment but not yet shipped from the licensee's premises shall be maintained by the licensee by an internal record system such that the licensee is constantly aware of the material's location and the zM

proposed time of shipment. Individuals who are involved in the shipping of such material and/or the storage of such material prior to shipment, shall be trained in the precautions necessary for such handling and storage.
C.
For material which has decayed to background levels as determined by radioassay or external level as measured with appropriately calibrated instruments, records shall indicate that the material was determined to be no longer radioactive and will indicate the methods and results of the survey or analysis.
D.
Shipment records of radioactive waste material shall be maintained and the licensee shall require written confirmation from the authorized recipient of such material that this material has been received.
E.
All records and written confirmations required by this condition shall be maintained for inspection by the Department.
- The requirements of this condition are in addition to any other requirements for the handling and/or disposal of radioactive material contained in this license and " State Regulations for Protetion Against Radiation".
- 20. The individuals listed below are the only persons authorized by this license to act as radiographers or radiographer's assistants as defined in 1200-2-4.04(1)(f f) and (gg), and 1200-2.05(1) of " State Regulations for Protection Against Radiation".
