The Attachment provides the Post Accident Monitoring (PAM) Report for the inoperability of one channel of the Post Accident Neutron Flux Function for greater than 30 days. Wolf Creek Generating Station Technical Specification (TS) 5.6.8, "PAM Report," requires the submittal of the report within 14 days after entry into Condition B of LCO 3.3.3, "Post Accident Monitoring (PAM) Instrumentation."

Condition B.1 of LCO 3.3.3 was entered on July 21, 2022, as noted by the Wolf Creek Generating Station (WCGS) Control Room logs when the inoperable channel of the Neutron Flux Function was determined to be unable to be restored to OPERABLE status within 30 days. Cond B of LCO 3.3.3 was entered three days into the 30-day Cond A restoration time. Due to the early entry into Cond B, this caused a PAM Report due date discrepancy at the station. Condition Report (CR 10017123) has been initiated to document the early entry into LCO 3.3.3 Cond B and the PAM Report due date discrepancy.

Attachment to WO 22-0021 Page 1 of 1 Technical Specification 5.6.8 - Post Accident Monitoring (PAM) Report Background Information:

Wolf Creek Generating Station (WCGS) declared one channel of the Post Accident Neutron Flux Function inoperable on July 18, 2022, at 1803 Central Standard Time (CST). This is due to a failed card in the pulse counting range circuit of one of the two Wide Range Post Accident Excore Detectors. WCGS Technical Specification (TS) 3.3.3, Post Accident Monitoring (PAM)

Instrumentation, Condition A was entered when one Neutron Flux required channel was determined to be inoperable. Required Action A.1 of TS 3.3.3 requires restoring the required channel to OPERABLE status within 30 days. If the Required Action and associated Completion Time of Condition A are not met, TS 3.3.3 Condition B is entered. On July 21, 2022, the station entered into LCO 3.3.3 Condition B. LCO 3.3.3 Required Action B.1 requires initiation of action in accordance with TS 5.6.8, PAM Report, and requires that a report be submitted within 14 days of entry into Condition B.

Note: Condition Report 10017123 was initiated as a result of an early entry into LCO 3.3.3 Cond B and a discrepancy of the required due date for the PAM Report submittal.

Preplanned Alternate Methods of Monitoring RCS Parameters:

Alternate means of monitoring containment radiation levels are available to operators in the Main Control Room while this channel (SE NI-60B) of Wide Range Post Accident Excore Detectors is INOPERABLE. The other channel of Post-Accident Wide Range (SE NI-61B) successfully swapped from the campbelling range to the pulse counting range and is indicating as expected.

Diverse indications (post-accident source range, Westinghouse Source Range, and Westinghouse Intermediate range) are all OPERABLE and are indicating as expected.

Cause of Inoperability:

Wolf Creek Generating Station (WCGS) declared one channel of Post-Accident Neutron Flux INOPERABLE on July 18, 2022, due to a failed card in the pulse counting range circuit of one of the two Wide Range Post Accident Excore Detectors. The detector failed off-scale low when power dropped below the campbelling range and entered the pulse-counting range.

Plans and Schedule for Restoring the Instrumentation Channels of the Function to Operable Status:

Troubleshooting was performed on the failed card in the Post Accident Wide Range and the card was found to be defective. Planning is ongoing to replace the failed card and an implementation schedule is being developed to install it.
