Dear Sir or Madam:

In accordance with ASME Section XI, Code Case N-532-4, "Repair/Replacement Activity Documentation Requirements and Inservice Summary Report Preparation and Submission," Indian Point Unit No. 3 herein submits the 2015 "Form OAR-1 Owners Activity Report." This report describes relevant conditions that required evaluation and repairs and replacements to be performed during the period April 2013 through April 2015.

F-A/Fl.20C 2-2550 SI-H- 15B-S CR-IP3-2015-01475 - Support had one nut loose. Nut tightened.

F-A/Fl.10A 1-4101 SI-H-351-2 CR-IP3-2015-01591 - Support was not carrying the weight of the pipe. Engineering evaluated the condition and determined that the support was operable in current condition.

F-A/F1.40 3-1100 32 Support CR-IP3-2015-01638 - 32 RHR Heat Exchanger top support; one of the tie rod supports was bent with an approximate 2" upward deflection. Engineering evaluated condition and determined that the support was operable in current condition.

E-A/E1.30 Moisture Barriers CR-IP3-2015-01888 - Corrosion was found on four (4) different locations of the containment liner. UT was performed on most deteriorated location. Engineering evaluated condition and determined that liner was operable in current condition.

E-A/E1.11 b Equipment/ Personnel Hatch CR-IP3-2015-01984 - Two equipment hatch bolts had engagement issues. Engineering evaluated this condition and determined that the bolting was operable in the current condition.
