SUMMARY

On June 28-30, 2016, an Ingestion Pathway Exercise was conducted for the Indian Point Energy Center (IPEC). On June 28 the plume portion focused on the 10-mile plume exposure pathway emergency planning zone (EPZ) around the Indian Point Energy Center (IPEC). On June 29, 2016, the ingestion portion focused on sampling, analysis and assessment activities and concluded with decision making. On June 30, 2016, a tabletop exercise focused on implementation of those decisions. The exercise was evaluated by the Federal Emergency Management Agency (FEMA), Region II. The purpose of the exercise was to demonstrate and assess the level of State and local preparedness in responding to a radiological emergency. The exercise was held in accordance with FEMA's policies and guidance concerning the exercise of State and local radiological emergency response plans (RERP) and procedures.

The most recent previous exercise at this site was conducted on October 7, 2014. The qualifying emergency preparedness exercise was conducted on March 3, 1982.

FEMA wishes to acknowledge the efforts of the many individuals in New York State; Westchester, Rockland, Orange, and Putnam Counties in New York; and Bergen County, New Jersey who participated in this exercise.

Protecting the public health and safety is the full-time job of some of the exercise participants and an additional assigned responsibility for others. Still others have willingly sought this responsibility by volunteering to provide vital emergency services to their communities. The cooperation and teamwork of all participants were evident during this exercise.

This report contains the draft evaluation of the biennial exercise and of the following out-of-sequence activities in Orange, Putnam, Rockland and Westchester counties: Day Cares and School Interviews; Emergency Worker Decontamination Centers; General and Special Population Bus Companies; School Bus Companies; Traffic Control Points; and Medical Services Drills.

The State and local organizations, except where noted in this report, satisfactorily demonstrated knowledge of their emergency response plans and procedures and adequately implemented them.

There were no Level 1 Findings. Four Level Two findings were corrected on the spot by re-demonstration.

SUMMARY

2.1 Exercise Purpose and Design The DHS/FEMA Region II Office evaluated the full participation, Ingestion Pathway Exercise on June 28-30 to assess the capabilities of the local emergency preparedness organizations in implementing their Radiological Emergency Response Plans and Procedures to protect the public health and safety during a radiological emergency involving the Indian Point Energy Center (IPEC). The purpose of this report is to present the results of the findings on the performance of the offsite response organizations during a simulated radiological emergency.

2.2 Exercise Objectives, Capabilities and Activities Capabilities-based planning allows for exercise planning teams to develop exercise objectives and observe exercise outcomes through a framework of specific action items that were derived from the Target Capabilities List (TCL). The capabilities listed below form the foundation for the organization of all FEMA Region II REP Program objectives and observations in this exercise.

• Emergency Operations Center (EOC) Management: Is the capability to provide multi-agency coordination (MAC) for incident management by activating and operating an EOC for a pre-planned or no-notice event. EOC management includes EOC activation, notification, staffing, and deactivation; management, direction, control, and coordination of response and recovery activities; coordination of efforts among neighboring governments at each level and among local, regional, state, and federal EOCs; coordination of public information and warning; and maintenance of the information and communication necessary for coordinating response and recovery activities.

• Emergency Public Information and Warning: Is the capability that includes public information, alert/warning and notification. It involves developing, coordinating, and disseminating information to the public, coordinating officials, and incident management and responders across all jurisdictions and disciplines effectively under all hazard conditions.

• Citizen Evacuation and Shelter in Place: Is the capability to prepare for, ensure communication of, and immediately execute the safe and effective sheltering-in-place of an at-risk population (and companion animals), and/or the organized and managed evacuation of the at-risk population (and companion animals) to areas of safe refuge in response to a potentially or actually dangerous environment. In addition, this capability involves the safe reentry of the population where feasible.

• Emergency Public Safety and Security Response: Is the capability to reduce the impact and consequences of an incident or major event by securing the affected area, including crime/incident scene preservation issues as appropriate, safely diverting the public from hazards, providing security support to other response operations and properties, and sustaining operations from response through recovery. Public Safety and Security Response requires coordination among officials from law enforcement (LE), fire, and Emergency Medical Services (EMS).

• Hazardous Materials Response and Decontamination: Is the capability to assess and manage the consequences of a hazardous materials release, either accidental or as part of a terrorist attack. It includes testing and identifying all likely hazardous substances onsite; ensuring that responders have protective clothing and equipment; conducting rescue operations to remove affected victims from the hazardous environment; conducting geographical survey searches of suspected sources of contamination and establishing isolation perimeters; mitigating the effects of hazardous

• materials, decontaminating on-site victims, responders, and equipment; coordinating off-site decontamination with relevant agencies, and notifying environmental, health, and law enforcement agencies having jurisdiction for the incident to begin implementation of their standard evidence collection and investigation procedures.

• Mass Care: Is the capability to provide immediate shelter, feeding centers, basic first aid, bulk distribution of needed items, and related services to persons affected by a large-scale incident, including special needs populations. Special needs populations include individuals with physical or mental disabilities who require medical attention or personal care beyond basic first aid. Other special-needs populations include non-English speaking populations that may need to have information presented in other languages. The mass care capability also provides for pet care/handling through local government and appropriate animal-related organizations. Mass care is usually performed by nongovernmental organizations (NGO), such as the American Red Cross 18

Special- needs populations are generally the responsibility of local government, with medical needs addressed by the medical community and/or its alternate care facilities.

State and Federal entities also play a role in public and environmental health by ensuring safe conditions, safe food, potable water, sanitation, clean air, etc.

• Triage and Pre-Hospital Treatment: Is the capability to appropriately dispatch EMS resources; to provide feasible, suitable, and medically acceptable pre-hospital triage and treatment of patients; to provide transport as well as medical care en route to an appropriate receiving facility; and to track patients to a treatment facility.

0830 Unit 2 commences plant shutdown.

0840 Unit 2 SM declares an Alert based on EAL FA 1.1 "Any loss or any potential loss of either Fuel Clad or RCS (Table F-1)", based on: FC loss #E4 (1-131 >300 uCi/gm) 0925 ERO facilities are operational.

0955 21 Reactor Coolant Pump (RCP) High Vibration condition. Unit 2 CCR attempts to manually trip the Rx which is unsuccessful from the CCR console. Unit 2 has automatic turbine trip. 21 RCP High Vibration condition causes a small (>6 gpm) RCS leak inside the VC. VC Radiation monitors begin rising due to RCS leak.

Day 3: Tabletop End of Emergency Event Day 1, Tuesday, June 28, 2016 Brief The following information for this phase summarizes many of the activities and events which occurred following the event. The information represents a collection of status reports, media reports and verbal communications.

Unique equipment out of service for the Exercise:

The Unit 2 -21 Auxiliary Boiler Feed Pump was out of service for Bearing Oil leak repair and the Unit 2-21 Safety Injection (SI) pump was out of service for outboard mechanical seal replacement.

Basic Accident (Exercise) Summary During enhanced Reactor Coolant System (RCS) sampling the Unit 2 watch chemist reported a ~350uCi/gm activity level. The Unit 2 Shift Manager declared an Alert based on Emergency Action Level (EAL) FA 1.1 There are indications of a loss or potential loss to either the Fuel Cladding or Reactor Coolant System Integrity The 21 Reactor Coolant Pump (RCP) developed a High Vibration condition. The Unit 2 Control Room (CCR) attempted to manually trip the nuclear reactor, which was unsuccessful from the CCR console. Unit 2 received an automatic turbine trip removing power from the grid. The 21 RCP High Vibration condition caused a small (>6 gallon per minute) RCS leak inside the domed Vapor Containment building.

The Emergency Director declared a Site Area Emergency (SAE), based on EAL SS2.1 The reactor control rods have failed to shut down the reactor The 23 Steam Generator (S/G) developed a primary to secondary tube leak of 400 gpm. The 23 S/G Safety Valve automatically opened and remained stuck open. This caused an unfiltered radiological release to atmosphere.

The Emergency Director declared a General Emergency (GE) based on EAL FGI.1 Loss of 2 of 3 fission product barriers with actual or potential loss of the third barrier After 2.2 hours2.314815e-5 days <br />5.555556e-4 hours <br />3.306878e-6 weeks <br />7.61e-7 months <br />, the primary RCS pressure decreased, lowering the flow associated with the primary to secondary tube leak. The radiation levels inside the Auxiliary Boiler Feed Pump building (location of the 23 S/G Safety Valve) decreased to the point that members of a Repair and Corrective Action (R&CA) team were cycled in to make repairs. The 23 S/G Safety valve was closed and the radiological release to atmosphere was secured.

Indian Point Energy Center Emergency/Recovery Actions:

The Indian Point Entergy Center Unit 2 is shutdown; the on-site impact of the accident is still being assessed. The Operations personnel continue to transition Unit 2 towards cold shutdown.

The Emergency Response Organization began transitioning into the Emergency Termination &

Recovery Mode and began to develop long-term recovery plans in conjunction with increased NRC oversight.

The NRC Site Team (Incident Response Team) is fully functional. The NRC set up a field office in the Emergency Operation Facility (EOF).

The relief Indian Point ERO is being bussed onto the site with State Police escort from the Jefferson Valley Mall in Westchester County NY. An on-site event review meeting was conducted with off-going key ERO members.

Unit 2 Operations personnel placed the plant in cold shutdown on the Residual Heat Removal system. Habitability in the Unit 2 Control Room has been verified satisfactory and is being used to control the shutdown Unit 2 reactor.

The relief Indian Point ERO team developed a Recovery Plan Outline. A formal discussion with regulatory, State and local authorities was conducted to ensure coordinated entry into recovery.

After Action Report/Improvement Plan Indian Point Energy Center Emergency Event, Day 2-7, July 5, 2016 Brief The following information summarizes the activities which occurred on day 2 through day 7 of the event.

IPEC Plant Status:

The Event Summary Report for the event was generated on June 29, 2016 and was distributed to appropriate regulatory, State and local authorities at 0700.

The Indian Point Energy Center Unit 2 is in cold shutdown on the Residual Heat Removal system. The 23 S/G Safety Valve remains shut and there is no chance of additional radiological releases.

Unit 2 remains in the Recovery Phase. The on-site OSC/TSC Emergency Response facilities are no longer required and the site has transitioned into a repair outage with personnel being bussed onto the site for 12 hour1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br /> shifts from the Cortlandt State Police barracks. Extensive radiological surveys have been performed onsite and the Unit 2 Auxiliary Boiler Feed Pump building and portions of the North end of the Owner Controlled property, remain contaminated. All other areas of the site have been released from radiological controls. Additional assets including Radiological Personnel, Westinghouse Personnel, Corporate personnel, Entergy personnel from other nuclear sites and additional cooling water have all arrived on site. A large presence of NRC personnel remain onsite and have set-up a new field office in the Technical Support Center (TSC) facility.

Entergy Radiological Environmental Monitoring Teams are deployed for continued sampling and boundary verification of ground deposition. All samples have been split and are being sent to Knox Lab and Brookhaven National Laboratories.

No entries into the Unit 2 domed Vapor Containment building have been made.

A Root Cause Analysis of the event is in progress with a full report expected in 30 days.

The County and State Emergency Operation Centers (EOC) are still open on a limited scale. The JIC has shut-down at Hawthorne and transfers media operations to the State EOC. Media briefings have been held throughout the week, the last one was completed at 0600.

Off-Site Emergency/Recovery Actions:

On July 28, 2016, Governor Andrew M. Cuomo declared a State Disaster Emergency for Westchester, Putnam, Rockland, Orange Counties and contiguous areas in response to the incident at Indian Point. This helped to expedite emergency response services.

State agencies and non-profit organizations have gathered at the State Emergency Operations Center in Albany, to support the States response for this incident. State officials are also at the plants Emergency Operating Facility and the surrounding counties Emergency Operations Centers.

After Action Report/Improvement Plan Indian Point Energy Center Governor Andrew M. Cuomo has also designated Commissioner John P. Melville, Chairman of the New York State Disaster Preparedness Commission (DPC), as the State Coordinating Officer for this incident. Mr. Melville serves as the Commissioner of the Division of Homeland Security

& Emergency Services (NYSDHSES).

New York State has activated the NYS Helpline for New Yorkers who need information or assistance in those communities affected by the incident. New Yorkers can call the NYS Helpline for various types of assistance from various State and other agencies, including volunteer organizations such as the American Red Cross (ARC) and Salvation Army (SA), as well as important health and safety tips.

In coordination with the U.S. Coast Guard (USCG), the State has cleared the Hudson River -

from the Tappan Zee Bridge to the Newburgh-Beacon Bridge - of all boat traffic.

The New York State Office of Parks, Recreation and Historic Preservation (OPRHP) have closed all State parks in the affected area.

The Metropolitan Transportation Authority (MTA) - has suspended the Metro-North Commuter Railroad (MNCR) Hudson Line.

The New York State Division of Military & Naval Affairs (DMNA) have closed and secured the Camp Smith Training Site in the Town of Cortlandt, Westchester County.

Federal agencies responding to the New York State EOC are the Federal Emergency Management Agency (FEMA), United States Department of Agriculture (USDA), Food and Drug Administration (FDA), Health and Human Services (HHS), Environmental Protection Agency (EPA), DOE, NRC, and the Centers for Disease Control (CDC). In addition, the United States Department of Energy (DOE) and the Environmental Protection Agency (EPA) has activated the Federal Radiological Monitoring Assistance Center (FRMAC). The FRMAC Advance Party has arrived at the Emergency Operations Facility (EOF) at the Indian Point power plant and is working with Indian Point and state agency representatives. It has been determined that the FRMAC will be located at the New York Air National Guard Base at Stewart International airport. The Department of Energy has also been asked to complete a serpentine flyover at low altitudes to determine areas of radiological deposition.

Representatives from the New York State Department of Health (DOH), the Wadsworth Center Laboratory, and the State Radiological Field Teams are working with federal agencies to determine the areas of concern.

Cuomo initially urged farmers within a 50-mile radius of Indian Point to take precautionary actions to protect food supplies as a result of the incident at Indian Point the plant. The State Department of Agriculture and Markets has issued recommendations to area farmers:

• Place all livestock on stored feed and water and shelter them if possible.

• Cover feed if stored outdoors.

• Store as much water as possible and cover wells, rain barrels and water tanks.

• Cover any harvested produce or place it inside.

After Action Report/Improvement Plan Indian Point Energy Center This recommendation applies to all types of farms, including commercial, non-commercial, and recreational. All milk, soil and crops within the affected area will be monitored and sampled for possible contamination, and results will be reported to the public as soon as possible.

The New York Division of State Police (DSP) is coordinating security with other nuclear power plants and other critical infrastructures, and is assisting with traffic control in the affected area.

Staff from the New York State Department of Healths (DOH) Bureau of Environmental Radiation Protection and the Public Health Preparedness Program was deployed to the State Emergency Operations Center. The State DOH is also staffing Emergency Worker Personnel Monitoring Centers (EWPMC) in coordination with the State University of New York (SUNY),

NYS Department of Labor, NYS Department of Environmental Conservation and the NYS Division of Homeland Security & Emergency Services.

The New York State Department of Agriculture and Markets reports that area farmers were advised earlier to place livestock on stored feed and grain. Food safety inspectors and dairy products specialists have been alerted and are standing by.

Governor Andrew M. Cuomo has directed the Department of Agriculture and Markets to restrict the sale or distribution of agricultural products from approximately a 50 mile area downwind of the plant. This will reduce public risk by keeping potentially contaminated fresh food products from entering the market. All agricultural products, including dairy, livestock, poultry, eggs, fruits, vegetables and forage, within this area may not be eaten, distributed, transported, or sold.

Livestock and poultry should remain on stored and covered feed and water.

Governor Andrew M. Cuomo directed the State Department of Health to conduct comprehensive testing of public water supplies in areas surrounding Indian Point.

Boundaries of evacuated areas are being patrolled by State and local law enforcement as well as the New York National Guard.

The New York State and federal ingestion pathway sampling teams have conducted extensive sampling throughout the 50 mile ingestion zone for the Indian Point Energy Center. An analysis of these samples has indicated the need to take more specific protective actions.

FEMA, NYSDHSES Recovery and State Human Service Agencies have opened a Disaster Recovery Center (DRC) Field Office in Poughkeepsie, New York (Dutchess County). The state and federal agencies are working together to organize information on damage assessment.

Damage assessment will continue to be slow until the restricted zone is established and areas for return have been identified. In addition to state and federal agencies, representatives from American Nuclear Insurers (ANI) will be available to process claims.

After Action Report/Improvement Plan Indian Point Energy Center Monitoring of all evacuees has been completed and reception centers continue to be staffed for congregate care. Reception centers will remain open until long-term needs of all evacuees are identified and met.

Westchester and Putnam Counties have requested assistance from the state for decontamination activities. These Emergency Worker Personnel Monitoring Centers (EWPMC) equipped to support decontamination are located at the following New York State Department of Transportation (NYSDOT) facilities: 106 Ludington Road, Holmes, N.Y. (Putnam County), 35 Walker Road Valhalla, N.Y. (Westchester County) and 228 Townline Road, West Nyack, N.Y.

(Rockland County). These continue to be opened during the event. The New York State Department of Health continues to track emergency worker exposure results from those state and county agency personnel working in or near the evacuated zones.

The NYS Department of Transportation has notified the Federal Aviation Administration (FAA) to relax the air restriction that was imposed over the plume emergency planning zone during the emergency phase. However, the restrictions remain in force for the area over the facility site as well as the restricted zone.

No significant damage has occurred to the utilities (water, electric, gas) in the evacuated areas.

Weather Conditions:

Therefore, 44 CFR Part 350 approval of the offsite radiological emergency response plans and preparedness for the State of New York, site-specific to the Indian Point Energy Center, will remain in effect.

1. At the New York State Emergency Operations Center, the NYS Public Service Commission representative demonstrated an in depth technical understanding of power plant operations.

She was able to accurately diagnosis the accident and release pathway. She did an outstanding job using "layman's terms" to explain the loss of fission product barriers and release pathway to the executive group and EOC personnel.

2. The Lead Public Information Officer (PIO) for the JIC demonstrated excellent direction and control. He had many responsibilities in which he was able to excel, even though the tasks at hand were numerous. The Lead PIO initiated both a PIO Coordination Line and the Virtual Teleconference (VTC) to keep communications open with the state and county EOCs. These tools were also used to conduct briefings and provide coordination prior to media briefings.

Additionally, the PIO kept abreast of public inquires and rumors by using the PIO Coordination Line to monitor and check on the phone bank locations. Confirming proper information, coordinating briefings and dispelling all rumors kept the Lead PIO very busy, but he effectively juggled all duties and assured that briefings went smoothly and all information going out was correct.

3. Once Bergen County (BC) realized the number of evacuees that might be coming from Rockland County, there was some concern about whether there would be enough Radiation Monitoring Equipment and Potassium Iodine (KI) for the evacuees heading to the Bergen County Community College Reception Center. The BC logistic Chief and the New Jersey State Police Emergency Management discussed options for getting additional equipment and resources to the facility as quickly as possible. Their discussion proved to be valuable and prudent, because instead of acquiring the equipment from their 1st MOU choice, the logistic chief learned from NJSP that it would be better to receive the Radiation Monitoring Equipment and KI from New Jersey, Ocean County OEM, where they would have enough portal monitors to supply the facility and trained and certified personnel to operate the equipment.

4. During Day 2 ingestion sampling, the Field Team Coordinator used a status board to keep each team on track in preparing for and completing sampling tasks.

5. During Day 2 ingestion sampling, the Red Team read procedures quickly but efficiently during sampling tasks to make sure procedures were adhered to. Also, one team member voluntarily declared her pregnancy and the FTC staff knew to contact NYS Health and obtain special exposure control guidelines. A scale was utilized to make sure appropriate quantity of sample was collected.

SUMMARY

On June 28-30, 2016, an Ingestion Pathway Exercise was conducted for the Indian Point Energy Center (IPEC). On June 28 the plume portion focused on the 10-mile plume exposure pathway emergency planning zone (EPZ) around the Indian Point Energy Center (IPEC). On June 29, 2016, the ingestion portion focused on sampling, analysis and assessment activities and concluded with decision making. On June 30, 2016, a tabletop exercise focused on implementation of those decisions. The exercise was evaluated by the Federal Emergency Management Agency (FEMA), Region II. The purpose of the exercise was to demonstrate and assess the level of State and local preparedness in responding to a radiological emergency. The exercise was held in accordance with FEMA's policies and guidance concerning the exercise of State and local radiological emergency response plans (RERP) and procedures.

The most recent previous exercise at this site was conducted on October 7, 2014. The qualifying emergency preparedness exercise was conducted on March 3, 1982.

FEMA wishes to acknowledge the efforts of the many individuals in New York State; Westchester, Rockland, Orange, and Putnam Counties in New York; and Bergen County, New Jersey who participated in this exercise.

Protecting the public health and safety is the full-time job of some of the exercise participants and an additional assigned responsibility for others. Still others have willingly sought this responsibility by volunteering to provide vital emergency services to their communities. The cooperation and teamwork of all participants were evident during this exercise.

This report contains the draft evaluation of the biennial exercise and of the following out-of-sequence activities in Orange, Putnam, Rockland and Westchester counties: Day Cares and School Interviews; Emergency Worker Decontamination Centers; General and Special Population Bus Companies; School Bus Companies; Traffic Control Points; and Medical Services Drills.

The State and local organizations, except where noted in this report, satisfactorily demonstrated knowledge of their emergency response plans and procedures and adequately implemented them.

There were no Level 1 Findings. Four Level Two findings were corrected on the spot by re-demonstration.

SUMMARY

2.1 Exercise Purpose and Design The DHS/FEMA Region II Office evaluated the full participation, Ingestion Pathway Exercise on June 28-30 to assess the capabilities of the local emergency preparedness organizations in implementing their Radiological Emergency Response Plans and Procedures to protect the public health and safety during a radiological emergency involving the Indian Point Energy Center (IPEC). The purpose of this report is to present the results of the findings on the performance of the offsite response organizations during a simulated radiological emergency.

2.2 Exercise Objectives, Capabilities and Activities Capabilities-based planning allows for exercise planning teams to develop exercise objectives and observe exercise outcomes through a framework of specific action items that were derived from the Target Capabilities List (TCL). The capabilities listed below form the foundation for the organization of all FEMA Region II REP Program objectives and observations in this exercise.

• Emergency Operations Center (EOC) Management: Is the capability to provide multi-agency coordination (MAC) for incident management by activating and operating an EOC for a pre-planned or no-notice event. EOC management includes EOC activation, notification, staffing, and deactivation; management, direction, control, and coordination of response and recovery activities; coordination of efforts among neighboring governments at each level and among local, regional, state, and federal EOCs; coordination of public information and warning; and maintenance of the information and communication necessary for coordinating response and recovery activities.

• Emergency Public Information and Warning: Is the capability that includes public information, alert/warning and notification. It involves developing, coordinating, and disseminating information to the public, coordinating officials, and incident management and responders across all jurisdictions and disciplines effectively under all hazard conditions.

• Citizen Evacuation and Shelter in Place: Is the capability to prepare for, ensure communication of, and immediately execute the safe and effective sheltering-in-place of an at-risk population (and companion animals), and/or the organized and managed evacuation of the at-risk population (and companion animals) to areas of safe refuge in response to a potentially or actually dangerous environment. In addition, this capability involves the safe reentry of the population where feasible.

• Emergency Public Safety and Security Response: Is the capability to reduce the impact and consequences of an incident or major event by securing the affected area, including crime/incident scene preservation issues as appropriate, safely diverting the public from hazards, providing security support to other response operations and properties, and sustaining operations from response through recovery. Public Safety and Security Response requires coordination among officials from law enforcement (LE), fire, and Emergency Medical Services (EMS).

• Hazardous Materials Response and Decontamination: Is the capability to assess and manage the consequences of a hazardous materials release, either accidental or as part of a terrorist attack. It includes testing and identifying all likely hazardous substances onsite; ensuring that responders have protective clothing and equipment; conducting rescue operations to remove affected victims from the hazardous environment; conducting geographical survey searches of suspected sources of contamination and establishing isolation perimeters; mitigating the effects of hazardous

• materials, decontaminating on-site victims, responders, and equipment; coordinating off-site decontamination with relevant agencies, and notifying environmental, health, and law enforcement agencies having jurisdiction for the incident to begin implementation of their standard evidence collection and investigation procedures.

• Mass Care: Is the capability to provide immediate shelter, feeding centers, basic first aid, bulk distribution of needed items, and related services to persons affected by a large-scale incident, including special needs populations. Special needs populations include individuals with physical or mental disabilities who require medical attention or personal care beyond basic first aid. Other special-needs populations include non-English speaking populations that may need to have information presented in other languages. The mass care capability also provides for pet care/handling through local government and appropriate animal-related organizations. Mass care is usually performed by nongovernmental organizations (NGO), such as the American Red Cross 18

• Triage and Pre-Hospital Treatment: Is the capability to appropriately dispatch EMS resources; to provide feasible, suitable, and medically acceptable pre-hospital triage and treatment of patients; to provide transport as well as medical care en route to an appropriate receiving facility; and to track patients to a treatment facility.

The Unit 2 -21 Auxiliary Boiler Feed Pump was out of service for Bearing Oil leak repair and the Unit 2-21 Safety Injection (SI) pump was out of service for outboard mechanical seal replacement.

Basic Accident (Exercise) Summary During enhanced Reactor Coolant System (RCS) sampling the Unit 2 watch chemist reported a ~350uCi/gm activity level. The Unit 2 Shift Manager declared an Alert based on Emergency Action Level (EAL) FA 1.1 There are indications of a loss or potential loss to either the Fuel Cladding or Reactor Coolant System Integrity The 21 Reactor Coolant Pump (RCP) developed a High Vibration condition. The Unit 2 Control Room (CCR) attempted to manually trip the nuclear reactor, which was unsuccessful from the CCR console. Unit 2 received an automatic turbine trip removing power from the grid. The 21 RCP High Vibration condition caused a small (>6 gallon per minute) RCS leak inside the domed Vapor Containment building.

The Emergency Director declared a Site Area Emergency (SAE), based on EAL SS2.1 The reactor control rods have failed to shut down the reactor The 23 Steam Generator (S/G) developed a primary to secondary tube leak of 400 gpm. The 23 S/G Safety Valve automatically opened and remained stuck open. This caused an unfiltered radiological release to atmosphere.

The Emergency Director declared a General Emergency (GE) based on EAL FGI.1 Loss of 2 of 3 fission product barriers with actual or potential loss of the third barrier After 2.2 hours2.314815e-5 days <br />5.555556e-4 hours <br />3.306878e-6 weeks <br />7.61e-7 months <br />, the primary RCS pressure decreased, lowering the flow associated with the primary to secondary tube leak. The radiation levels inside the Auxiliary Boiler Feed Pump building (location of the 23 S/G Safety Valve) decreased to the point that members of a Repair and Corrective Action (R&CA) team were cycled in to make repairs. The 23 S/G Safety valve was closed and the radiological release to atmosphere was secured.

Indian Point Energy Center Emergency/Recovery Actions:

The Indian Point Entergy Center Unit 2 is shutdown; the on-site impact of the accident is still being assessed. The Operations personnel continue to transition Unit 2 towards cold shutdown.

The Emergency Response Organization began transitioning into the Emergency Termination &

Recovery Mode and began to develop long-term recovery plans in conjunction with increased NRC oversight.

The NRC Site Team (Incident Response Team) is fully functional. The NRC set up a field office in the Emergency Operation Facility (EOF).

The relief Indian Point ERO is being bussed onto the site with State Police escort from the Jefferson Valley Mall in Westchester County NY. An on-site event review meeting was conducted with off-going key ERO members.

Unit 2 Operations personnel placed the plant in cold shutdown on the Residual Heat Removal system. Habitability in the Unit 2 Control Room has been verified satisfactory and is being used to control the shutdown Unit 2 reactor.

The relief Indian Point ERO team developed a Recovery Plan Outline. A formal discussion with regulatory, State and local authorities was conducted to ensure coordinated entry into recovery.

After Action Report/Improvement Plan Indian Point Energy Center Emergency Event, Day 2-7, July 5, 2016 Brief The following information summarizes the activities which occurred on day 2 through day 7 of the event.

IPEC Plant Status:

The Event Summary Report for the event was generated on June 29, 2016 and was distributed to appropriate regulatory, State and local authorities at 0700.

The Indian Point Energy Center Unit 2 is in cold shutdown on the Residual Heat Removal system. The 23 S/G Safety Valve remains shut and there is no chance of additional radiological releases.

Unit 2 remains in the Recovery Phase. The on-site OSC/TSC Emergency Response facilities are no longer required and the site has transitioned into a repair outage with personnel being bussed onto the site for 12 hour1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br /> shifts from the Cortlandt State Police barracks. Extensive radiological surveys have been performed onsite and the Unit 2 Auxiliary Boiler Feed Pump building and portions of the North end of the Owner Controlled property, remain contaminated. All other areas of the site have been released from radiological controls. Additional assets including Radiological Personnel, Westinghouse Personnel, Corporate personnel, Entergy personnel from other nuclear sites and additional cooling water have all arrived on site. A large presence of NRC personnel remain onsite and have set-up a new field office in the Technical Support Center (TSC) facility.

Entergy Radiological Environmental Monitoring Teams are deployed for continued sampling and boundary verification of ground deposition. All samples have been split and are being sent to Knox Lab and Brookhaven National Laboratories.

No entries into the Unit 2 domed Vapor Containment building have been made.

A Root Cause Analysis of the event is in progress with a full report expected in 30 days.

The County and State Emergency Operation Centers (EOC) are still open on a limited scale. The JIC has shut-down at Hawthorne and transfers media operations to the State EOC. Media briefings have been held throughout the week, the last one was completed at 0600.

Off-Site Emergency/Recovery Actions:

On July 28, 2016, Governor Andrew M. Cuomo declared a State Disaster Emergency for Westchester, Putnam, Rockland, Orange Counties and contiguous areas in response to the incident at Indian Point. This helped to expedite emergency response services.

State agencies and non-profit organizations have gathered at the State Emergency Operations Center in Albany, to support the States response for this incident. State officials are also at the plants Emergency Operating Facility and the surrounding counties Emergency Operations Centers.

After Action Report/Improvement Plan Indian Point Energy Center Governor Andrew M. Cuomo has also designated Commissioner John P. Melville, Chairman of the New York State Disaster Preparedness Commission (DPC), as the State Coordinating Officer for this incident. Mr. Melville serves as the Commissioner of the Division of Homeland Security

& Emergency Services (NYSDHSES).

New York State has activated the NYS Helpline for New Yorkers who need information or assistance in those communities affected by the incident. New Yorkers can call the NYS Helpline for various types of assistance from various State and other agencies, including volunteer organizations such as the American Red Cross (ARC) and Salvation Army (SA), as well as important health and safety tips.

In coordination with the U.S. Coast Guard (USCG), the State has cleared the Hudson River -

from the Tappan Zee Bridge to the Newburgh-Beacon Bridge - of all boat traffic.

The New York State Office of Parks, Recreation and Historic Preservation (OPRHP) have closed all State parks in the affected area.

The Metropolitan Transportation Authority (MTA) - has suspended the Metro-North Commuter Railroad (MNCR) Hudson Line.

The New York State Division of Military & Naval Affairs (DMNA) have closed and secured the Camp Smith Training Site in the Town of Cortlandt, Westchester County.

Federal agencies responding to the New York State EOC are the Federal Emergency Management Agency (FEMA), United States Department of Agriculture (USDA), Food and Drug Administration (FDA), Health and Human Services (HHS), Environmental Protection Agency (EPA), DOE, NRC, and the Centers for Disease Control (CDC). In addition, the United States Department of Energy (DOE) and the Environmental Protection Agency (EPA) has activated the Federal Radiological Monitoring Assistance Center (FRMAC). The FRMAC Advance Party has arrived at the Emergency Operations Facility (EOF) at the Indian Point power plant and is working with Indian Point and state agency representatives. It has been determined that the FRMAC will be located at the New York Air National Guard Base at Stewart International airport. The Department of Energy has also been asked to complete a serpentine flyover at low altitudes to determine areas of radiological deposition.

Representatives from the New York State Department of Health (DOH), the Wadsworth Center Laboratory, and the State Radiological Field Teams are working with federal agencies to determine the areas of concern.

Cuomo initially urged farmers within a 50-mile radius of Indian Point to take precautionary actions to protect food supplies as a result of the incident at Indian Point the plant. The State Department of Agriculture and Markets has issued recommendations to area farmers:

• Place all livestock on stored feed and water and shelter them if possible.

• Cover feed if stored outdoors.

• Store as much water as possible and cover wells, rain barrels and water tanks.

• Cover any harvested produce or place it inside.

After Action Report/Improvement Plan Indian Point Energy Center This recommendation applies to all types of farms, including commercial, non-commercial, and recreational. All milk, soil and crops within the affected area will be monitored and sampled for possible contamination, and results will be reported to the public as soon as possible.

The New York Division of State Police (DSP) is coordinating security with other nuclear power plants and other critical infrastructures, and is assisting with traffic control in the affected area.

Staff from the New York State Department of Healths (DOH) Bureau of Environmental Radiation Protection and the Public Health Preparedness Program was deployed to the State Emergency Operations Center. The State DOH is also staffing Emergency Worker Personnel Monitoring Centers (EWPMC) in coordination with the State University of New York (SUNY),

NYS Department of Labor, NYS Department of Environmental Conservation and the NYS Division of Homeland Security & Emergency Services.

The New York State Department of Agriculture and Markets reports that area farmers were advised earlier to place livestock on stored feed and grain. Food safety inspectors and dairy products specialists have been alerted and are standing by.

Governor Andrew M. Cuomo has directed the Department of Agriculture and Markets to restrict the sale or distribution of agricultural products from approximately a 50 mile area downwind of the plant. This will reduce public risk by keeping potentially contaminated fresh food products from entering the market. All agricultural products, including dairy, livestock, poultry, eggs, fruits, vegetables and forage, within this area may not be eaten, distributed, transported, or sold.

Livestock and poultry should remain on stored and covered feed and water.

Governor Andrew M. Cuomo directed the State Department of Health to conduct comprehensive testing of public water supplies in areas surrounding Indian Point.

Boundaries of evacuated areas are being patrolled by State and local law enforcement as well as the New York National Guard.

The New York State and federal ingestion pathway sampling teams have conducted extensive sampling throughout the 50 mile ingestion zone for the Indian Point Energy Center. An analysis of these samples has indicated the need to take more specific protective actions.

FEMA, NYSDHSES Recovery and State Human Service Agencies have opened a Disaster Recovery Center (DRC) Field Office in Poughkeepsie, New York (Dutchess County). The state and federal agencies are working together to organize information on damage assessment.

Damage assessment will continue to be slow until the restricted zone is established and areas for return have been identified. In addition to state and federal agencies, representatives from American Nuclear Insurers (ANI) will be available to process claims.

After Action Report/Improvement Plan Indian Point Energy Center Monitoring of all evacuees has been completed and reception centers continue to be staffed for congregate care. Reception centers will remain open until long-term needs of all evacuees are identified and met.

Westchester and Putnam Counties have requested assistance from the state for decontamination activities. These Emergency Worker Personnel Monitoring Centers (EWPMC) equipped to support decontamination are located at the following New York State Department of Transportation (NYSDOT) facilities: 106 Ludington Road, Holmes, N.Y. (Putnam County), 35 Walker Road Valhalla, N.Y. (Westchester County) and 228 Townline Road, West Nyack, N.Y.

(Rockland County). These continue to be opened during the event. The New York State Department of Health continues to track emergency worker exposure results from those state and county agency personnel working in or near the evacuated zones.

The NYS Department of Transportation has notified the Federal Aviation Administration (FAA) to relax the air restriction that was imposed over the plume emergency planning zone during the emergency phase. However, the restrictions remain in force for the area over the facility site as well as the restricted zone.

No significant damage has occurred to the utilities (water, electric, gas) in the evacuated areas.

1. At the New York State Emergency Operations Center, the NYS Public Service Commission representative demonstrated an in depth technical understanding of power plant operations.

She was able to accurately diagnosis the accident and release pathway. She did an outstanding job using "layman's terms" to explain the loss of fission product barriers and release pathway to the executive group and EOC personnel.

2. The Lead Public Information Officer (PIO) for the JIC demonstrated excellent direction and control. He had many responsibilities in which he was able to excel, even though the tasks at hand were numerous. The Lead PIO initiated both a PIO Coordination Line and the Virtual Teleconference (VTC) to keep communications open with the state and county EOCs. These tools were also used to conduct briefings and provide coordination prior to media briefings.

Additionally, the PIO kept abreast of public inquires and rumors by using the PIO Coordination Line to monitor and check on the phone bank locations. Confirming proper information, coordinating briefings and dispelling all rumors kept the Lead PIO very busy, but he effectively juggled all duties and assured that briefings went smoothly and all information going out was correct.

3. Once Bergen County (BC) realized the number of evacuees that might be coming from Rockland County, there was some concern about whether there would be enough Radiation Monitoring Equipment and Potassium Iodine (KI) for the evacuees heading to the Bergen County Community College Reception Center. The BC logistic Chief and the New Jersey State Police Emergency Management discussed options for getting additional equipment and resources to the facility as quickly as possible. Their discussion proved to be valuable and prudent, because instead of acquiring the equipment from their 1st MOU choice, the logistic chief learned from NJSP that it would be better to receive the Radiation Monitoring Equipment and KI from New Jersey, Ocean County OEM, where they would have enough portal monitors to supply the facility and trained and certified personnel to operate the equipment.

4. During Day 2 ingestion sampling, the Field Team Coordinator used a status board to keep each team on track in preparing for and completing sampling tasks.

5. During Day 2 ingestion sampling, the Red Team read procedures quickly but efficiently during sampling tasks to make sure procedures were adhered to. Also, one team member voluntarily declared her pregnancy and the FTC staff knew to contact NYS Health and obtain special exposure control guidelines. A scale was utilized to make sure appropriate quantity of sample was collected.
