Special Report of a Degraded Fire Barrier Resulting from Personnel Negligence During a routine plant tour on May 13, 1987, a Fire Protection Coordinator discovered damage to a fire barrier wall in the entrance to the Radiation Access Control Area (RACA). The metal lath and plaster wall was found to have two gouges, each approximately four inches long by two inches wide. The wall, which encloses redundant electrical cables related to the safe shutdown capability of the plant, consists of two panels separated by an air space to achieve a three hour fire rating. Only the exterior panel was damaged.

The plant was in a cold shutdown condition at the time of the discovery.

The degraded condition of the fire barrier wall in Fire Area CC made it inoperable and the action statement of Technical Specification 3.7.10.a was entered. An hourly fire watch patrol was already in effect in Fire Area CC for other identified fire protection deficiencies (Licensee Event Report 86-034). A Potential Condition Adverse to Quality Report was issued, prompting the initiation of corrective maintenance. All of the required materials were not available,however, and repair of the wall has been delayed until their receipt.

The wall was probably damaged as a result of personnel negligence while transporting equipment in RACA. More specific identification of cause cannot be deduced from the general nature of the gouges and plant management has not been able to identify the specific event or personnel responsible for this damage. The fire barrier wall has been in place for approximately two years with no damage reported until this incident. The fire barrier wall is a 8711300132 R 870612 3 ADO K O 6 S g _LL

This appears to be an isolated incident. It is estimated that repairs to the wall will be completed by July 31, 1987.

This report is submitted in accordance with Technical Specifications 3.7.10.a and 6.9.2, which require a 30 day Special Report whenever a fire barrier is inoperable for greater than.seven consecutive days.
