Special Report of: an Inoperable Fire Detection Zone Resulting from a Failed Detector in that Zone-1 On July'18, 1987 at 0700 hours, fire detection zone'FDZ-A208 was declared- ) inoperable. The required. fire watch hourly patrol:of-the zone ~ panel was immediately. established:in:accordance with Technical Specification'3.3.3.'8, action: statement "a". This fire' detection zone' monitors the 590 foot' elevation in the Shield Building annulus.. When:the FDZ-A208 alarm was' received,lthe:areaf j was surveyed, and no source of-fire or smoke'vas.found. JIt,vas determined that. ,1 one of the ten fire detectors (Allison, A1200 series ionization. detector) that lj make up FDZ-A208 had failed. With this detettor' inoperable'the zone; alarm i remains in alarm. Once the zone alarm is actuated, alarms from additional-detectors in that zone are received at the. local' zone panel but do not re-actuate the zone alarm into the control room.- Maintenance'Vork Order (MVO) 1-87-2273-00. vas initiated on July'24,.1987fto. repair the failed detector. Due to the' detector's location in a high radiation i the HWO vill;be worked during an outage of sufficient duration-to permit-

area, scaffolding erection without excessive worker exposure'(planned.for the 5th refueling outage, approximately February 1988)..The detector will be examined' at that time to investigate the cause of its failure.

1 The hourly fire watch will continue until this detector / zone is returned to an operable status. On Oc ober 7, 1987, the Fire Protection Group noted that-the' fire protection. zone panel C-2720 had been in alarm since July 18, 1987 and the required'Special-Report had not'been submitted. Technical Specification 3.3.3.8 action! statement- '"b" requires a Special Report within 30 days after the fire detection zone had been inoperable for 14 days, h K-00 6 s

Personnel did not initiate a Potential' Condition 1 Adverse to Quality Report (PCA0R) at the time FDZ-A208 was declared inoperable. i The Station's PCAQR system tracks the relevant action. statements'and informs'the appropriate group of the potential Special Report. 'The Shift Supervisor ~ made his Unit Log entry indicating the zone was inoperable 1 and noted that the' hourly fire'vatch patrol'was established.- However,-he did I not-initiate a PCAQR. The Nuclear Group Procedure NG-0A-702 requires the -{ initiation of a PCA0R for an event potentially " reportable to anfoutside agency". As corrective action for the late Special Report, all Shift Supervisors vill be required to review this incident to-improve their understanding of the proper mechanism for ensuring such incidents are reviewed and reported appropriately.
