License No. SNM-1227 Richland,LW'ashington' 99352 As a result of. t'he -inspection conducted July _21-25,.1986, and in ac'cordance

- with NRC Enforcement Policy 10 CFR'Part 2 Appendix C,. the following violation was identified:

License Condition 9' states, the-licensee shall conduct their program in

- accordance with_ statements, representations'and conditions-contained in the

. license. application dated.May 31, 1979, as supplemented by letters to the

. Commission. Section 2.3.'4.a)',of the' license application requires preparation and use of-detailed work and operating procedures and training of employees.

Procedure No. XN-NF-P91, 001 Revision 1, " General Facility Radiation Work Procedure", page 1 requires that workers inform HPT personnel whenever-2 is

-visible ~or smearable contamination in excesslof 10,000 dpm/100 cm known o'r, suspected to exist outside of'a ventilated enclosure within a contamination control area.

Contiary tothis requirement, at the time of.the inspection, an -

' electrician;while^ performing equipment maintenance at the North end of the.Line 2;calciner inside the contamination control area'of-the UOz.

Building,1 spilled uncontained visible UO2 Powder which was not reported by the worker to HPT; This is.la Severity Level IV-Violation;(Supplement VI).

' Notice, a' written statement or explanation in reply, including:

(1) the

corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further items of noncompliance; and (3) th'e date when full compliance will be achieved. Consideration may be given to.cxtending your response time for good cause shown.
