APPENDIX A NOTICE OF VIOLATION Exxon Nuclear Company, Inc. Docket No, 70-1257 2101 Horn Rapids Road License No. SNM-1227 Richland, Washington 99352 .

As a result of the inspection conducted February 3-7 and 20, 1986, and in accordance with NRC Enforcement Policy 10 CFR Part 2 Appendix C, the following violations were identified:

License Condition 9 states, the licensee shall conduct their program in accordance with s'atements, t representations and conditions contained in the license application dated May 31, 1979, as supplemented by letters to the Commission. Section 2.3.4.a) of the license application requires preparation and use of detailed work and operating procedures and training of employees.

A. Procedure No. P43,078 Revision 0, " Inspection and Testing of UF cylinders"section7.5requiresthattheairtestofcylindervklvesbe conducted at 100 psig, and that the valve threads be. checked for leaks at this pressure. Also, this test pressure is consistent with ANSI N14.1, 1982.

Contrary to this requirement, at the time of the inspection, the licensee's operational test procedure used after replacement of faulty valves on UF cylinders in the conversion room, specified that valves be 6

tested between 70 and 90 psig.

This is a Severity Level IV Violation (Supplement VI).

B. Procedure No. P66,336 Revision 18 " Preparing and Removing UF 6 Cylinders -

Line 1" section 6.0 requires that the licensee shall put the protective cover for the valve in place prior to hookup of the lifting device, and also section 6.6.4 requires that the licensee shall place the protective cover on the valve prior to installing the lifting yolk.

Contrary to the above requirement, a protective valve cover on cylinder number EX827 was not used when removing it from a vaporization chest in Line 1 Conversion Room on February 4, 1986.

C. Procedure No. P43,078 Revision 0, " Inspection and Testing of UF 1 cylinders" section 7.8 Equipment Winterization requires that thh licensee shall' remove the hose reels, crate, and store them in the warehouse and also that the licensee shall lock the piping cabinet.

Contrary to the above requirement, at the time of the inspection, the hose reels used for testing the UF cylinders had not been winterized in 6

that the reels were located outside without any shelter from the winter weather. Also, the piping cabinet was not locked.

The above two items constitute a Severity Level V Violation (Supplement VI).

Pursuant to the provisions of 10 CFR 2.201, Exxon Nuclear Company Inc. is hereby required to submit to this office within thirty days of the date of this Notice, a written statement or explanation in reply, including: (1) the corrective steps which have been tak.en and the results achieved; (2) corrective steps which will be taken to avoid further items of noncompliance; and (3) the date when full compliance will be achieved. Consideration may be given to extending your response time for good cause shown.
