SUBJECT:

POTENTIAL GENERIC QUESTION AND INPUT TO THE VENDOR INSPECTION PROGRAM ON A PART 21 REPORT AND LICENSEE QA AND RELATED TO LIMITORQUE On August 27, 1986, a meeting was held with Portland General Electric Company (PGE) concerning'a Quality Assurance (QA) audit that.was done in early August of

.Limitorque's procurement of Peerless Motors (Attached meeting summary report).

This meeting identified two items of interest.

First, a Part 21 Report and followup (see attached meeting summary repo'rt Enclosures 4 and 5) identified a potentially generic question. . That is, a'certain combination of a varnish-finish over a type of insulation should'not

'be used since failures of motors containing this combination have been observed. This failure potential for safety-related valves may be an appropriate topic for an Information Notice and Vendor Program Branch inspection because.the situation probably exists at other plants. Limitorque has not, to our knowledge, comununicated this situation to other users of similarly manufactured motors.

Secondly, PGE's findings in their QA audit were quite specific (See attached Meeting Summary Report). These findings have been communicated to the Vendor Program Branch for their consideration in their upcoming audit.
