1 MEMORANDUM FOR: Carl J. Paperiello, Director, Division of Reactor Safety FROM: Charles E. Norelius, Director, Division of Reactor Projects

SUBJECT:

REQUEST FOR ELECTRICAL INSPECTION AT PALISADES During the month of August 1986 numerous electrical system problems occurred /were discovered at Palisades. Most notably these involved failures of diesel generator (DG) breakers to close upon demand due to maintenance activities in progress, a failure of the power supply breaker to vital bus 1-E to close upon demand on August 26, and the discovery of a design deficiency in the DC distribution system such that ECCS is rendered vulnerable to a single failure.

5 By receipt of this memorandum you are requested to perform an electrical inspection at Palisades to review events described above and, as a minimum, answer the following questions:

1. Was the electrical lineup which defeated auto closure of the DG output l

breaker in support of maintenance the optimal lineup or could a lineup have been established which would have allowed the output breaker to

remain operable?
2. Did electrical maintenance personnel adequately interface with operations personnel in establishing the electrical lineup to support maintenance such that the operators were fully aware of the impact of that lineup on this electrical system?

3. Has the licensee determined the root cause of the problem with the supply breaker to the 1-E bus, and, if so, has the licensee reviewed this cause for in-plant generic effect and implemented component' inspections or augmented preventive maintenance / testing?
4. Is the licensee's assessment of the single failure in the DC distribution system correct? Are proposed modifications acceptable? Has the licensee implemented a program to explore the in-plant generic aspects of this finding?
