4 NOTICE OF VIOLATION Toledo Edison Company Docket No. 50-346 As a result of the inspection conducted on January 5, through March 3, 1986, 1 and in accordance with the " General Policy and Procedures for NRC Enforcement Actions," 10 CFR Part 2, Appendix C, (1985), the following violation was identified:

1. 10 CFR 50, Appendix B, Criterion II, " Quality Assurance Program,"

requires a quality assurance program which complies with the requirements of Appendix B and is documented by written policier, procedures, or instructions and shall be carried out. The Nuclear Quality Assurance Manua'l, Section 2, which implements 10 CFR 50, Appendix B, Criterion II, Item 19, states that Toledo Edison complies with the requirements of ANSI N18.7-1972. ANSI N18.7-1972 Paragraph 5.3.9 requires that procedures be provided for the control of materials and components necessary fcr plant maintenance and modification.

Toledo Edison nuclear safety-related Procedure AD1847.00, Station Material Control, requires that excess material be returned to the storeroom within 30 days of job completion.

Contrary to the above, three components were found in the custody of the station maintenance department which were assigned to MW0s that had been completed for greater than 30 days.

This is a Severity Level V violation (Supplement I).

2. 10 CFR 50.71 requires that annual Final Safety Analysis Report (FSAR) revisions shall reflect all changes to the facility up to a maximum of six months prior to the date of filing.
a. Contrary to the above, the July 1984, annual revision of the FSAR was found not to reflect an extensive series of modifications to the pressurizer code safety relief valves completed on July 30, 1982.
b. Contrary to the above, in February of 1986, following the July 1985 annual submittal, the FSAR was found not to reflect an extensive facility modification to the service water pump room's ventilation l

system completed on September 11, 1984.
