APPENDIX A NOTICE OF VIOLATION TU Electric Dockets: 50-445/88-59 50-446/88-55 Comanche Peak Steam Electric Station Permits: CPPR-126 Units 1 and 2, Glen Rose, Texas CPPR-127 During a NRC inspection conducted on August 3 through September 8, 1988, a violation of NRC requirements was identified. In accordance with the "General Statenient of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1988), the violation is listed below:

Criterion V of Appendix B to 10 CFR Part 50, as implemented by Section 5.0, Revision 3 of the TU Electric Quality Assurance Plan (QAP), states, in part, "Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, or a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings . . . ."

Instructions for Attachment 5, "Flex Conduits," Revision 3, to Field Verification Method (FVM) CPE-SWEC-FVM-EE/ME/IC/CS-089 states, in part, "All flex conduit . . . will be verified for proper type, only the unsat flex conduit will be listed on data sheet."

contrary to the above, the following conditions were identified for j room 150A:

1. Conduit C03G10245 was listed on the data shoot as Amortito i Type VF (Unsatisfactory) though it is actually Sca3 tite Type UA (Satisfactory).
2. Conduit C03G06656 was not listed though it is Amortite Type VF (Unsatisfactory).

This is a Severity Level IV violation (Supplement II)

2 Inspection Programs, Comanche Peak Project Division, Office of Special Projects, within 30 days of the date of the letter transmitting this Notice. This reply should be clearly marked as a "Reply to a Notice of Violation" and should include for each violation: (1) the reason for the violation if admitted, (2) the corrective steps that have been taken and the results achieved, (3) the corrective steps that will be taken to avoid further violations, and (4) the date when full compliance will be achieved.

If an adequate reply is not received within the time specified in this Notice, an order may be issued to show cause why the license should not be modified, suspended, or revoked or why such other action as may be proper should not be taken. Where good cause is shown, consideration will be given to extending the response time.
